摘要
目的调查驻琼部队泌尿系结石的流行病学特征,分析其相关危险因素。方法 2012-06~11月,驻琼部队3580名男性军人接受B超筛查和问卷调查。对泌尿系结石患者进行尿比重检测,随机抽取200名无结石官兵进行尿比重检测作为对照。应用t检验、χ2检验及logistic回归分析对所得数据进行统计学处理。结果 3580人全部进行了B超检查并有效回收全部调查问卷。泌尿系结石整体发病率为7.29%,其中海军部队泌尿系结石发病率为6.85%,陆军部队发病率为7.84%,差异无统计学意义(P>0.05)。泌尿系结石患者尿比重与对照组相比显著增高(P<0.05)。户外作训强度大(Ⅲ级以上)、作训环境温度>27℃、湿度>60%、以及潜艇工作环境、饮水少(<1500 ml/d)与驻琼部队官兵泌尿系结石发生率密切相关(P<0.05)。结论泌尿系结石在驻琼官兵中的发病率较高,高温、湿热环境,高强度作训而饮水量较少是导致该地区部队作训人员易发生泌尿系结石的独立危险因素。注意适时增加饮水量,对维持驻琼部队特殊军事作业环境中人体水的摄入量与损失量平衡、预防泌尿系结石的发生至关重要。
Objective To study the epidemiologmal characteristcs and analyse the related risk factors of urinary calculus in Hainan forces. Methods Between 2012 June and 2012 November, 3580 male soldiers in Hainan troops accepted Bultrasonic examination and completed the questionnaires. The urinary specific gravity was tested for the subjects with urinary calculus and 200 randomly selected normal controls. The Student's t test, test and logistic regression analyses were used to analysis the data. Results All the 3580 subjects accepted B-ultrasonic examination and all the questionnaires were effective. The overall incidence rate of urinary calculus was 7. 29%, 6. 85% in navy and 7. 84.0%o in ground force respectively, and the difference was not Statistically significant (P^0. 05). The urinary specific gravity was significantly higher in the subjects with urinary calculus than that in the control group (P%0. 05). Several factors were closely related to the high morbidity of urinary calculus in Hainan troops, including high intensity of the outdoor work, environmental temperature higher than 27℃, humidity higher than 60%, submarine work environment and not enough water intake (% 1500 ml/d). Conclusion The incidence rate of urinary calculus in Hainan troops is relatively high. The hot and humid environment, high work intensity and not enough water intake are the independent risk factors. Paying attention to increase the water intake amount is critical to maintain the balance of water intake and loss for the soldiers working in the specific milltray ,work envirnnment and prevent the occurrenc of urinary calculus.
