摘要
目的应用单心动周期实时三维超声心动图(sRT-3DE)评价房间隔缺损患者治疗前后右心室收缩功能及其影响因素。资料与方法对30例房间隔缺损患者和30例健康志愿者行常规及sRT-3DE检查,比较房间隔缺损患者治疗前后右心室相关参数的改变,分析治疗前右心室射血分数变化的相关因素及sRT-3DE测得的右心室射血分数与常规二维超声心动图测得的右心室面积变化分数的相关性。结果与健康志愿者比较,房间隔缺损患者右心室舒张末容积、收缩末容积、每搏输出量增加(P<0.01),射血分数减低(P<0.01);治疗后,患者右心室舒张末容积、收缩末容积、每搏输出量明显减小(P<0.01),但射血分数变化不明显(P>0.05)。房间隔缺损治疗前右心室射血分数与右心室收缩末容积、舒张末容积呈显著正相关(R=0.975,P<0.001);sRT-3DE测得的右心室射血分数与右心室面积变化分数呈显著正相关(r=0.792,P<0.05)。结论 sRT-3DE能准确评价房间隔缺损患者治疗前后右心室收缩功能,患者右心室射血分数减低主要为右心室收缩末容积及舒张末容积增加所致。
Purpose To assess the right ventricular systolic function and the influence factors before and after treatment of atrial septal defect (ASD) by single cardiac cycle real-time three-dimensional echocardiography (sRT-3DE) in patients with ASD. Materials and Methods Routine examination and sRT-3DE were pertbrmed on thirty patients with ASD and thirty normal subjects, and the right ventricular parameters were obtained. The parameter changes was compared and the related influenced factors of right ventricular ejection fraction (RVEF) was analyzed before and alter the treatment, and the correlation was evaluated between the RVEF and fraction of area change (FAC). Results The right ventricular end-diastolic volume (RVEDV), the right ventricular end-systolic volume (RVESV), and the right ventricular stroke volume (RVSV) were higher (P〈0.01), and the RVEF was lower (P〈0.01) in the ASD group (before treatment) than that in the normal group. After treatment, the RVEDV, RVESV, RVSV in patients with ASD were lower than those before treatment (P〈0.01), but the RVEF had no significant change (P〉0.05). RVEF was positively correlated with RVEDV and RVESV (R=0.975, P〈0.001); RVEF was positively correlated with FAC using sRT-3DE (r-0.792, P〈0.05). Conclusion sRT-3DE can accurately evaluate the right ventricular contraction of patients with ASD belbre and after treatment, the decrease of RVEF attributes to the increase of RVESV and RVEDV of ASD.
