实时三维超声心动图评价房间隔缺损封堵治疗前后右心功能的研究 被引量:2

摘要 目的运用实时三维超声心动图(RT-3DE)研究房间隔缺损封堵治疗前后右心功能变化情况。方法入选继发孔型房缺患者33例,均于封堵治疗前后行经胸实时三维超声心动图(RT-3D-TEE)检查,检测其右心室功能相关指标进行分析。结果房间隔缺损(ASD)患者在行封堵治疗术后右心室舒张末期内径(RVEDd)、右心室舒张末期容积(RVEDV)、右心室收缩末期容积(RVESV)均较术前明显缩小(P<0.01);而右室面积变化分数(RVFAC)及三维右室射血分数(3D-RVEF)在封堵术后较术前明显降低(P<0.01),但在术后3个月下降趋缓,术后6个月达到新的平衡。结论①房间隔缺损患者在行封堵治疗后右心功能可得到不同程度的改善;②实时三维超声心动图(RT-3DE)评价右心功能具有便捷、无创、可重复性强、敏感性和准确性高等优点,能较准确全面地评价右心功能的情况。 Objective To investigate the changes of fight heart function of patients with atrial septal defect closure before and after treatment by using real-time three-dimensional eehocardiogranhy ( RT-3DE ). Methods A total of 33 patients diagnosed ostium secundum ASD were selected. All of the patients were checked through transthoracic real-time three- dimensional eehocardiography (RT-3D-TEE). Then we detected and analysed the indexes related to fight ventricular function before and after closure treatment. Results The indexes of ASD patients such as fight ventricular end diastolic diameter (RVEDd), right ventricular end-diastolic volume (RVEDV) and right ventricular end-systolic volume (RVESV) reduced obviously (P 〈 0.01) then those before closure treatment. While right ventrieular fractional area change (RVFAC) and three-dimensional right ventricular ejection fraction (3D-RVEF) of the patients after transcatheter closure were lower than those before operation (P 〈 0.01 ). But after operated for 3 months it slowed down, and for 6 months achieved a new balance. Conclusion (1)The fight heart function of ASD patients can be improved in different degree after interventional therapy. (2)Real-time three-dimensional eehoeardiography (RT-3 DE) evaluating for the right heart function is convenient, noninvasive, easily repeatable, sensitive, accurate and advanced. It can evaluate for the right heart function accurately and comprehensively.
