胰岛素抵抗对急性ST段抬高型心肌梗死患者早期PCI治疗后心肌灌注及预后的影响 被引量:1

摘要 目的探讨胰岛素抵抗对急性ST段抬高型心肌梗死患者早期PCI治疗后心肌灌注及预后的影响情况。方法采用Judkins法行紧急冠状动脉造影和行急诊PCI治疗,自我平衡模型分析法(HOMA)指数(IRI)评价胰岛素抵抗。结果心肌灌注良好组术后6个月主要心血管事件(MACE)发生率均低于心肌灌注受损组,同时胰岛素抵抗组术后6个月主要心血管事件(MACE)发生率高于胰岛素敏感组,胰岛素抵抗组IRI和TMP分级呈现明显正相关,P<0.05,差异均有统计学意义。结论胰岛素抵抗影响了急性ST段抬高型心肌梗死患者早期PCI治疗后心肌灌注效果,影响了PCI近远期预后。
