感染实时监控系统在急诊科的应用价值 被引量:4

The value of real-time infection surveillance system (RT-INSS) in the emergency department

摘要 目的实现急诊科急危重病例感染的智能化识别预警与实时干预。方法对本科2011年全年1010例急危重患者入院后即应用感染实时监控系统实施动态感染指标的全程监控与预警。结果感染患者94人,感染发生率9.3%;感染部位前3位依次为下呼吸道50例(53.19%)、泌尿道感染13例(13.83%),血液感染13例(13.83%);细菌培养标本送检率为40.5%,检出鲍曼不动杆菌(14.65%),肺炎克雷伯菌(9.85%),铜绿假单胞菌(1.52%),白色念珠菌(11.23%),金黄色葡萄球菌(4.67%)等。结论应用实时感染监控系统对急诊科急危重病例进行感染的全程监测预警,及时发现危险因素,尽早采取积极有效的干预手段,合理使用抗菌药物,全面提升了急诊科急危重病例的感染预防、控制和管理水平,解决了急诊科感染实时监测问题。 Objective To solve the problem of intelligent identification, early warning and real-time intervention for infected critical illness in emergency department. Methods RT-INSS was started monitoring and process of nosocomial infections automatic identification and early warning indicators of infection once the 1010 critically ill patients in between January and December, 2011 were admitted. Results There are 94 cases of nosocomial (9.3%) in the department. The top three infections demonstrated 50 cases of respiratory tract (53.19%), 13 cases of urinary tract infection (13.83%), andl3 bloodstream infection (13.83%). The rate of send-in bacterial cultures of specimens was 40.5% , showed acinetobacter baumannii (14.65%), Klebsiella pneumonia (9.85%), pseudomonas aeruginosa (1.52%), candida albicans(11.23%) and staphylococcus aureus ( 4.67%), respectively. Conclusion The application of RT-INSS could achieve intelligent identification, early warning and real-time intervention for infected critical illness in emergency department and timely detection of risk factors and early intervention which is effective in reducing infection incidence, which can raise the overall level of nosocomial infection surveillance, prevention and control.
