摘要
目的 :检测纤维粘连蛋白 (FN)及其受体整合素 β1 亚单位在子宫内膜癌组织中的表达 ,探讨与组织分级和临床分期的关系以及在子宫内膜癌转移浸润中的意义。方法 :用免疫组织化学技术和图像分析法检测 5 8例子宫内膜腺癌、8例正常内膜组织中 FN和 β1 整合素的表达。结果 :正常子宫内膜腺上皮细胞 FN弱阳性表达 ,主要分布于细胞基底部 ;内膜腺癌组织 FN染色增强 ,明显高于正常对照组 (P<0 .0 1) ;低分化腺癌染色强度低于高分化组 (P<0 .0 5 ) ,阳性面积比例与细胞分级成反比。 β1 整合素在正常子宫内膜腺上皮呈强阳性表达 ;在不同分化的内膜腺癌组织表达均明显减少 (P<0 .0 1) ;与细胞分化有关 ;FN和 β1 整合素的表达与临床分期无关。结论 :子宫内膜腺癌组织 FN和 β1 整合素表达与正常明显不同。抑制肿瘤细胞的转移应采用调节粘附蛋白和粘附分子表达的策略。
Objective:To detect the expression of fibronectin FN and its receptor β 1 integrin in the tissue of carcinoma of endometrium and explore the relation between the tissue types and the clinical stages,and the significance in the expression in invasive metastasis of carcinoma of the endomerium.Methods:The expression of FN and β 1 integrin in 58 endometrial adenocarcinomas and the normal tissue of the endometrium in 8 cases were determined by means of immunohistochemistry and the pathological image analysis system.Results:There was a weakly positive expression of FN in the normal epithelium of the endometrium which was mainly distributed on the basilar membrane. Staining of FN is increased in the well differentiated carcinoma tissue that was significantly higher than that of the normal controls ( P< 0.01 ).The intensity of staining of the poorly differentiated carcinoma is significantly lower than that of the well differentiated one ( P<0.01 ).The positive area was manifested in an inverse proportion to the cell differentiation.There was a positive expression of β 1 integrin in the epithelium cells of the normal endometrium, and a significantly decreasing expression in the tissues of the differentiated carcinoma ( P<0.01 ) .It was related to cell differentiation.The expressions of FN and β 1 integrin are not related to the clinical stages.Conclusion:There was a significant difference in expression of FN and β 1 integrin between the endometrial adenocarcinoma and the normal endometrium tissue.The regulation of the expressions of FN and adhesive molecules is advisable to inhibit metastasis of carcinoma.
