摘要
评价心电图 (ECG)肢体导联室壁激动时间 (VAT)诊断左室肥厚 (L VH )的价值。方法 以超声心动图 (Echo)为标准 ,对 96例高血压患者左室重量 (L VM)进行测定 ,将邻近心电轴之肢体导联VAT与 Echo测定结果进行比较并分析其诊断 L VH的价值。结果 (1) L VM与 ECG心电轴邻近之肢体导联 VAT有极显著正相关 (r=0 .87及 0 .73,P<0 .0 1) ,(2 )以肢体导联 VAT≥ 0 .0 4s作为 L VH诊断指标与 Echo- L VH诊断标准比较 ,两种方法检测结果差异无显著性 (P>0 .0 5 ) ;(3)肢体导联 VAT≥ 0 .0 4s诊断 L VH的敏感性显著高于常规 ECG标准 (82 .4%和 2 1.6~ 5 2 .9% ,P<0 .0 0 5~ 0 .0 0 1)。结论 肢体导联 VAT对高血压 L VH的诊断有重要意义。
To evaluate the ventricular activation time(VAT) in limb leads for detecting left ventricular hypertrophy (LVH).Methods Using the echocardiography(Echo)as the validity standard,the left ventricular mass (LVM)were calculated in 96 patients with essential hypertension.The VAT close to the electrical axis were compared with echocardiographic criteria and analyzed for their value in detecting LVH.Results (1)the VAT in limb leads close to electrical axis was highly correlated with LVM documented by Echo(r=0 87 and 0 73, P <0 01),(2)there was no statistically significant difference between VAT≥0 04s and Echo criterion for detection of LVH( P >0 05),and(3)the sensitivity of VAT≥0 04s in limb leads was significantly greater than that found for usual electrocardiographic criteria (82 4% vs.21 6~52 9%, P <0 005~0 001).Conclusion The VAT in limb leads is of significant for the detecting of LVH in patients with hypertension.
