静脉注射胺碘酮对急性心肌缺血大鼠血流动力学的影响 被引量:2

Effect of Intravenous Administration of Amiodarone on The Hemodynamics in Rats with Acute Myocardial Ischemic

摘要 目的观察静脉应用胺碘酮对急性心肌缺血大鼠血流动力学的影响。方法健康雄性SD大鼠40只,体重250~300g,采用随机数字表法,将大鼠随机分为五组:假手术组(S组);缺血再灌注组(I/R组):胺碘酮2.5mg.kg-1+持续泵注组(A1组);胺碘酮5.0mg.kg-1组(A2组),不进行持续泵注;胺碘酮5.0 mg.kg-1组+持续泵注组(A3组)均于缺血前10min静脉注射胺碘酮。分别记录缺血前10min(T0),缺血后30min(T1),再灌注30min(T2),再灌注60min(T3),再灌注120min(T4)大鼠心率(HR),左心室收缩压(LVSP)、左心室终末舒张压(LVEDP)、左心室内压最大上升速率(+dp/dtmax)、左心室内压最大下降率(-dp/dtmax)的变化以及心律失常的分析。结果各组大鼠在T0时HR、LVSP、LVEDP、+dp/dtmax、-dp/dtmax比较差异无统计学意义(P>0.05);在缺血以后,与S组比较,I/R、A1、A2、A3组大鼠的HR、LVSP、+dp/dtmax、-dp/dtmax降低(P<0.05),LVEDP升高(P<0.05);与I/R组比较,A1、A2、A3组HR、LVSP、+dp/dtmax、-dp/dtmax升高(P<0.05),LVEDP降低(P<0.05);A1组HR、LVSP、+dp/dtmax、-dp/dtmax比A2、A3组降低,LVEDP升高(P<0.05),A3组HR、LVSP、+dp/dtmax、-dp/dtmax比A2组升高,LVEDP降低(P<0.05),组间比较差异有统计学意义(P<0.05)。各组大鼠在急性缺血期心律失常的评分显示,I/R、A1、A2、A3组评分均高于S组(P<0.05);A1、A2、A3组评分均低于I/R组(P<0.05);A2、A3组低于A1组,A3组低于A2组,差异均有统计学意义(P<0.05)。结论静脉应用胺碘酮可以改善急性心肌缺血大鼠的血流动力学及减少心律失常的发生率,以5.0mg.kg-1加持续泵注组效果最佳。 Objective To investigate the effect of intravenous administration of amiodarone on the hemodynamics in rats with acute myocardial ischemic.Methods 40 SD rats,weighting 250~300g,were randomly divided into 5 groups(n=8): sham-operation(S),ischemic-reperfusion control group(I/R),amiodarone 2.5mg·kg-1(A1),amiodarone5.0mg·kg-1(A2),aniodarone5.0mg·kg-1 with continuous pump injection were intravenous amiodarone in pre-ischemic 10min.At 30-min pre-inchemic(T0),ischemic(T1),30-min reperfusion(T2),60-min reperfusion(T3),120-min repefusion(T4)left ventricular systolic pressure(LVSP),left bentricular and diastolic pressure(LVEDP),+dp/dtmax and-dp/dtmax were measured.Results HR,LVSP,LVEDP,+dp/dtmax,-dp/dtmax showed no significant difference among groups at T0(P0.05);compared with S group,I/R group,A1group,A2group,A3 group's LVSP,+dp/dtmax,-dp/dtmax were reduced significantly(P0.05),menawhile LVEDP was increased significantly(P0.05);compared with I/R groups,A1group,A2group,A3 group 's HR,LVSP,+dp/dtmax,-dp/dtmax were increased significantly(P0.05),meanwhile LVEDP was reduced significantly(P0.05);compared with A2 group,A3 group,A1 group's HR,LVSP,+dp/dtmax,-dp/dtmax were reduced significantly and LVEDP was increased significantly(P0.05),and A3 group's HR,LVSP,+dp/dtmax,-dp/dtmax were increased significantly and LVEDP was reduced significantly(P0.05) compared with A2 group.The arrhythmia scores of acute ischemia displayed that I/R group,A1 group,A2 group,A3 group were significantly higher than S group(P0.05);A1 group,A2 group,A3 group were lower than I/R group(P0.05);A2 group,A3 group were significantly lower than A1 group,and A3 group was lower significantly than A2 group(P0.05).Conclusion Intravenous amiodarone can improve the hemodynamics in rats of acute myocardial ischemic and have the certain protective effect of myocardium.5.0 mg·kg-1 amiodarone with maintenance dose can reduce the arrhythmias induced by acute ischemia.
