摘要
目的 分析血清胃蛋白酶原(PG)I,II在胃癌前疾病及胃癌中的表达情况,探讨其在胃癌早期诊断中的价值.方法 选取2009年12月~2011年2月就诊的79例胃部疾病患者,根据胃镜检查及组织病理学结果分为四组.胃癌组32例,慢性萎缩性胃炎组20例,胃溃疡组27例,对照组48例均无消化道、肝脏、肾脏疾病及胃痛病史.采用酶联免疫法测定各组血清胃蛋白酶原I,II水平,采用化学发光法检测血清癌胚抗原(CEA)和糖类抗原(CA19-9)水平.结果 与对照组相比,胃癌组PGI降低(q=7.727,P〈0.01),PGI/PGII降低(q=6.839,P〈0.01),CEA升高(q=8.609,P〈0.01),CA19-9升高(q=9.127,P〈0.01);慢性萎缩性胃炎组PGI降低(q=5.371,P〈0.01),PGI/PGII降低(q=5.936,P〈0.01),CEA(q=0.764,P〉0.05)和CA19-9(q=1.174,P〉0.05)均无明显变化;胃溃疡组PGI升高(q=3.029,P〈0.05),PGII显著升高(q=5.045,P〈0.01),CEA(q=1.577,P〉0.05)和CA19-9(q=1.675,P〉0.05)均无明显变化.结论 血清胃蛋白酶原及其亚群在胃炎演变到胃癌的过程中,水平呈现不同的变化,相比传统肿瘤标志物仅在胃癌时水平升高,更能有效识别癌前疾病并对胃癌的风险进行评估,从而提高胃癌的早期诊断率.
Objective To analyze the expression of serum pepsinogen (PG) Ⅰ,Ⅱ in gastric cancer and gastric diseases,and to investigate the value in the early diagnosis of gastric cancer. Methods A total of 79 patients with stomach disease had been diagnosed from December 2009 to February 2011 ,and had been divided into four groups with the gastroscopy and tissue pathology results including 32 gastric cancer, 20 chronic atrophic gastritis, 27 gastric ulcer and 48 control with no gastrointestinal ,liver,kidney disease and stomach history. Enzyme immunoassay method was used to detect the serum pepsinogen Ⅰ and Ⅱ levels,and chemiluminescence method to detect the serum carcinoembryonic antigen (CEA) and carbohydrate antigen (CA19-9) level. Results Compared with the control,gastric cancer group PGⅠ reduced (q=7. 727 ,P〈0.01) ,PGⅠ/PGⅡ reduced (q=6. 839,P〈0. 01) ,CEA rise (q=8. 609,P〈0.01) ,CA19-9 rise (q=9. 127,P〈0.01) Chronic atrophic gastritis group PGⅠ reduced (q= 5.371, P〈0.01), PGⅠ/PGⅡreduced (q= 5. 936, P〈0. 01), CEA (q= 0.764, P〉0.05) and CA19-9 (q= 1. 174, P〉0. 05) were no obvious change; Gastric ulcer group PGI rise (q= 3. 029, P〈0. 05), PGIIsignificantly rise (q = 5. 045, P〈0.01) ,CEA (q= 1. 577, P〉0. 05) and CA19-9 (q= 1.675, P〉0. 05) were no obvious change. Conclusion The serum pepsinogen and its subsets level in the evolution from gastritis to gastric cancer could change differently. Compared with the traditional tumor markers only elevated in gastric cancer,it is helpful to effectively identify precancerous diseases, assess gastric cancer risk and improve early diagnosis of gastric cancer.
