摘要
目的通过对32例结肠癌性肠梗阻进行无肠道准备的磁共振成像研究,探讨无肠道准备的磁共振成像对结肠癌性肠梗阻的诊断价值。方法对44例结肠梗阻患者进行无肠道准备的磁共振多序列多方位扫描,重点加扫扩散加权序列;结合手术病理结果,对其中32例结肠癌性肠梗阻的磁共振表现进行综合分析。结果无肠道准备磁共振成像对本组32例结肠癌性肠梗阻的定位符合率为100%,对结肠癌的定性敏感度为100%,特异性为92.9%。32例结肠癌性肠梗阻表现为结肠癌以近肠管扩张积液或积粪,结肠癌肠壁表现为T1WI低信号及DWI明显高信号。结论无肠道准备的磁共振成像简便、安全、有效,可作为结肠癌性肠梗阻的重要检查手段。
Objective To evaluate the diagnostic value of MRI in colon carcinomatous ileus patients presenting with unprepared bowel by analyzing the MRI appearance of bowel obstructions caused by colon cancer.Methods Total 44 patients with obstructionofcolon were examined by multi-dimensional MRI with unprepared bowel,especially to carry out DWI scan;Together with surgical pathology results,the MRI appearance of bowel obstructions caused by colon cancer were analyzed synthetically.Results The accurate rate for obstruction location was 100%,the sensitivity and specificity in the diagnoses of colon cancer were 100% and 92.9%,respectively.32 cases of carcinomatous ileus showed proximal segment bowel expansion and hydrops or coprostasis,and colon cancer was presented as hyperintense signal in the T1-weighted and diffusion-weighting images.Conclusion The Unprepared bowel MRI can act as an important examination means to diagnose bowel obstructions caused by colon cancer;it is simple,safe and exact.
