摘要
目的研究比较维吾尔族(维族)与汉族女性乳腺癌分子分型特点,提高对乳腺癌分子分型的认识,更好地指导临床。方法采用2011St.Gallen最新乳腺癌分子分型方法,对170例经病理诊断证实的维、汉乳腺癌患者利用免疫组化及免疫荧光方法进行分子分型(Luminal A型、Luminal B型、HER-2过表达型和Basal-like型),结合临床病理资料,统计分析维族与汉族乳腺癌患者分子分型的异同点。结果维族与汉族患者的生育次数、肿瘤分级和肿瘤大小差异有统计意义;维族乳腺癌患者中组织学分级Ⅲ级所占比例高于汉族;维族乳腺癌患者在Her-2过表达型中年龄<35岁的比例高;在Luminal B型中月经状态为闭经和月经初潮年龄>15岁的维族患者比例高;在4种分子分型之间肿瘤的组织学分级差异有统计意义,HER-2过表达型和Basal-like型的患者倾向于高的组织学分级;在Luminal A型和Luminal B型中,维族乳腺癌患者的肿瘤直径大的比例高于汉族。结论维族与汉族乳腺癌的一般病理特性和分子分型存在一定的差异。维族乳腺癌患者具有一些预后差的特点。正确的认识不同民族间乳腺癌一般病理特性和分子分型的差异,对于临床乳腺癌的个体化治疗具有指导意义。
Objective To study and compared the differences about molecular subtype between Uygur and Han patients with breast cancer. Methods The new typing standard (2011 St. Gallen) was used to classi fy the molecular subtype of 170 breast caner cases. 4 immunohistochemical (IHC) markers (ER, PR, HER-2 and KI-67) were used to divide Uygur and Han breast cancer patients into 4 subtypes (Luminal A, Luminal B, HER-2 over expression and Basal-like), respectively. Then statistical analysis were used to evaluate difference in molecular subtype characteristics The Uygur patients had higher proportions of tumor HER-2 overexpression, age of menarche older than 15 by clinical and pathological characteristics. Results grade HI, age of onset less than 35 years in subtype years in subtype Basal-like and menopause in sub-type Luminal B. The tumor grade was different in 4 subtypes. The tumor size in Luminal A and Luminal B were different between Han and Uygur, Uygur patients had high proportion in large tumor. Conclusion There are some differences between Uygur and Compared with Han, Uygur patients had some Han in the pathological features and molecular subtypes. poor prognosis features. Correct understanding of breast cancer pathological characteristics and molecular typing differences between different ethnic, for clinical breast individualized treatment instructive.
