摘要
目的 探讨带旋髂深血管骨膜 骨瓣移植双头加压螺钉内固定治疗青壮年股骨颈骨折的疗效。方法 经髋关节Smith Peterson入路 ,取旋髂深血管束的髂骨骨膜 骨瓣移植于股骨颈骨折处骨槽内 ,加双头加压螺钉内固定治疗 34例。结果 随访 30例 ,随访时间 2～ 8年。 2 7例术后 4～ 6个月达骨性愈合 ,髋关节功能优良 ;3例股骨头缺血性坏死或骨折不连接。结论 带旋髂深血管的髂骨骨膜 骨瓣移植 ,血供充足 ,能促进骨折愈合 ,减少股骨头坏死和骨折不连接的发生 ,是治疗青壮年股骨颈骨折的有效方法。
Objective\ To explore the curative effects of femoral neck fracture in young adults by bi compression screw bundle of fixation and osteo periosteal bone grafts of deep iliac circumflex vessels. Methods\ 34 cases were treated by bi compression screws fixation and osteo periosteal bone grafts of deep iliac circumflex bundle of vessels in osteogroove of the femoral neck fracture with Smith Peterson incision. Results\ 30 cases were followed up for 2～8 years. 27 cases shew osseous union 4 to 6 months after operation,and the function of hip joint was excellent,3 cases had ischemic necrosis of femoral head or non union fracture. Conclusion\ The pedicled iliac osteo periosteal flap possesses adequate supply of blood. It can improve the unior of fracture,and reduce the rate of avascular necrosis of femur and non union.It is a valuable method to treat femoral neck fracture in young adults.
