摘要
目的 :探讨Joel Cohen切口子宫切除术的临床价值。方法 :于下腹部两髂前上棘连线下 2～ 3cm处做横行直切口 (即Joel Cohen切口 ) ,钝形分离脂肪层及腹直肌 ,纵向撕开腹膜 ,1～ 0华利康合成线连续缝合阴道残端 ,不缝合腹膜 ,1～ 0华利康合成线连续缝合筋膜 ,4号丝线间断缝合皮肤及皮下脂肪 3针。并与传统子宫切除术进行比较。结果 :Joel Co hen切口子宫切除术 ,手术时间短 ,术中出血量少 ,术后排气时间短 ,疼痛轻 ,术后发病率低 ,与对照组比较差异有显著性 (P<0 .0 5 )。结论 :Joel Cohen切口子宫切除术手术时间短 ,术后并发症少 ,住院时间短 。
Objective:To study the clinical effect of abdominal hysterectomy with Joel Cohen incision.Method:We made crossout straight incision under the interspinous line 2 3 centimeter on underside abdomen,separated subcutaneous tissue and m.rectus abdominis,tore peritonaeum lengthwise,stitched successively the remnants of vaginal with 1 0 Hualikang synthetic thread,the peritonaeum not stitched,then stitched fascia profunda in succession with 1 0 Hualikang synthetic thread,and stitched skin and subcutaneous tissue three stitches with 4 size silk thread.and compared with the traditional Abdominal Hysterectomy.Result:The abdominal hysterectomy with Joel Cohen incision can be accomplished with shorter operating duration,less bleeding,earlier anus exhaustion,less postoperative pain and lower incidence of postoperative morbidity.There were significant difference( P <0.05) between the two groups.Conclusion:The abdominal hysterectomy with Joel Cohen incision has shorter operading duration,fewer complications,shorter hospital stay,and therefore,it's suitable for wide spread use.
