摘要
目的 :探讨新式剖宫产 Joel- Cohen切口的临床价值。方法 :于下腹部双侧髂前上棘连线下大约 3cm处做横行直切口 ,钝性分离脂肪层及腹直肌 ,横向撕开腹膜 ,剖宫取胎 ,关腹时不缝合腹膜 ,用合成线连续缝合筋膜及间断全层缝合皮肤及皮下脂肪。并与同期子宫下段剖宫产术的中线垂直切口、横孤形切口比较。结果 :采用 Joel- Cohen切口的新式剖宫产手术时间短 ,术中出血量少 ,术后疼痛轻、病率低、愈合好 ,与两个对照组比较均有显著性差异 (P<0 .0 5)。结论 :新式剖宫产的 Joel- Cohen切口损伤小 ,出血少 ,术后恢复快 ,方法简单、易学 。
Objective:[WT5BZ]To explore the clinical value of Joel Cohen incision used in the Misgove Ladach method of cesarean section [WT5HZ]Methods:[WT5BZ]A transverse straight incision was performed 3cm below the level of anterior superior iliac spine at lower abdomen.the fat layer and musculus rectus abdominis were seperated bluntly,the peritoneum was torn transversely,the fetus was taken out after uterotomy,peritoneum wasnt sutured when closing abodomen,and then skin and subcutaneous tissue were sutured interruptedly with compound thread.We compared it with cesarean section with medius vertical and transveres are incision at lower abdomen at the same period.[WT5HZ]Results:[WT5BZ]The Misgove Ladach method of cesarean section had advantages of shorter operative time ,less blood loss during operation,slighter pain and lower sickness rate after operation,and better heal of incision.The difference was significant as compared with the two control groups (P<0.05).[WT5HZ]Conclusions:[WT5BZ]Joel Cohen incision in the Misgov Ladach method of cesarean section has significant clinical value for its advantages of less lesion in operation,less blood loss during operation,sooner recovery after operation and for it is simple and easy to grasp.[WT5HZ]
