摘要
目的探讨HA117、mdr1、mrp1、Irp、bcrp基因在AL患儿骨髓单个核细胞(BMMNC)中的表达及其在AL治疗中的临床意义。方法采用RT—PCR方法检测81例AL患儿BMMNC中耐药相关基因HA117、mdr1、mrp1、lrp,bcrp的表达;并以14例免疫性血小板减少性紫癜(ITP)患儿作为对照组。结果1.AL患儿HA117、mdr1、mrp1、lrp、bcrp阳性表达率均高于对照组,难治/复发组5种耐药相关基因的表达率均高于初诊组及缓解组。2.HA117、却阳性的首次完全缓解率(CR1)低于阴性的CR1率(P均〈0.05),mdr1、mrp1、bcrp阳性的CR1率与阴性的CR1率之间差异无统计学意义(P均〉0.05)。3.直线相关分析显示,AL中HA117与mdr1、mrp1、lrp、bcrp表达无相关性(r-0.031、0.319、0.203、0.176、11.178,P均〉0.05);mdr1与mrp1、mdr1与bcrp、却与bcrp呈正相关(r=0.260、0.308、0.317,P均〈0.05)。难治/复发组中,HA117与mdr1、mrp1、lrp及bcrp呈正相关。结论HA117与mdr1、mrp1、lrp、bcrp可能共同参与了儿童AL耐药的产生,可作为评估化疗效果与预后的一个重要指标。HA117与却较mdr1、mrp1及bcrp可能更具有预测多药耐药的意义。
Objective To explore the expressions of the HA117, mdrl,mrpl ,lrp and bcrp genes in bone marrow mononuelear cells (BMMNC)with acute leukemia (AL)and the clinical significance in AL. Methods Expressions of HA117, mdrl, mrpl ,lrp and bcrp genes in 81 children with AL and 14 children with idiopathic thrombocytopenic purpura (ITP) were tested using semi-quantitative reverse transcriptase polymerase chain reaction (RT-PCR) technique. Results 1. The expressions of HAll7 ,mdrl ,mrpl ,lrp and bcrp genes in AL patients were higher than that in ITP children. And the expressions of these genes in refractory-relapsed patients also higher than that in the initially diagnosed patients or remission patients. 2. The first complete remission (CR1)rate in HA117 and lrp positive cases was lower than that in negative cases( all P 〈0.05) ,but the CR1 rates in mdrl ,mrpl and bcrp positive cases had no significant difference from it in negative cases( all P 〉 0.05 ). 3. Correlation analysis showed:there was no correlation between the expression of HA117 and mdrl or HA117 and mrpl or HA117 and lrp or HA117 and bcrp in childhood AL( r = 0. 031, 0. 319,0. 203,0. 176,11. 178,all P 〉0.05). mdrl and mrpl or mdrl and bcrp or lrp and bcrp had obviously positive correlation (r = 0. 260,0. 308,0.317, all P 〈 0.05 ). In refractory-relapsed group, HA117 and mdrl or mrpl or lrp or bcrp had obviously perfect positive correlation. Conclusions The gene HA117 and mdrl, mrpl, lrp, bcrp may be asso- ciated with multidrug resistance of AL in children,therefore the detection of HA117 expression is helpful in management of AL patients. Comparing to the mdrl or mrpl or bcrp genes,the genes of HAll7 and Irp can better predict the multidrug resistance in childhood AL.
