舒芬太尼或(和)泮托拉唑预处理对浸水束缚应激大鼠胃黏膜损伤的影响 被引量:3

Effects of sulfentanyl and (or) pantoprazole pretreatment on acute gastric mucosal lesion induced by water immersion and restraint stress in rats

摘要 目的:观察舒芬太尼或(和)泮托拉唑预处理浸水束缚应激(WIRS)大鼠急性胃黏膜损伤的保护作用。方法:成年雄性Wistar大鼠30只,体重180~200g随机分成5组,正常对照组(NC组);应激对照组(WC组);泮托拉唑预处理组(PP组);舒芬太尼预处理组(SP组);舒芬+泮托拉唑预处理组(SPP组)每组6只,采用WIRS模型。WIRS6h后,切除全胃,检测胃液pH值、大体观察胃黏膜损伤程度、胃黏膜溃疡指数(GI)及光镜下观察组织学变化和H+-K+-ATP酶活性变化。结果:WC组WIRS 6h后大鼠的胃腔内见有大量咖啡色液体,黏膜明显的充血水肿,表面附有大量暗红色血痂,拭去血痂后可见点线状出血、甚至糜烂坏死;光镜下大鼠胃黏膜镜下细胞间隙增大,毛细血管扩张,上皮细胞有不同程度的坏死,有明显的中性粒细胞浸润SP、PP和SPPWIRS光镜下损伤显著减轻,PP和SPP较SP组显著,SPP组更明显;与NC组比,WIRS6h后,WC组大鼠胃黏膜GI显著增高,大鼠胃组织pH值和组织干湿比显著下降,H+-K+-ATP酶活性明显增高(P<0.01);SP、PP和SPP预处理可使GI显著下降,可抑制H+-K+-ATP酶活性,升高pH值,但PP和SPP更显著(P<0.01)。结论:舒芬太尼联合泮托拉唑比单独的舒芬太尼或单用泮托拉唑能更有效地预防应激状态下大鼠急性胃黏膜损伤,其联合应用没有排斥作用。 Objective To observe the effects of sulfentanyl and (or) pantoprazole pretreatment on acute gastric, mueosal lesion (AGML) induced by water immersion and restraint stress (WIRS) in rats. Methods 30 healthy male Wistar rats, weighted 180-220 g, were randomly divided into 5 groups (each n = 6): normal control group (Group NC), WIRS control group (Group WC), pantoprazole pretreatment group (Group PP), sulfentanyl pretreatment group (Group SP), sufentanyl and pantoprazole pretreatment group (Group SPP). 6 hours after WIRS, entire stomach was cut, then the pH of gastric fluid, the degree of gastric mucosa injury, and gastric mucosal injury index (GI) were detected. Histological changes were observed by optical microscope and changes of the activity of H+-K+-ATPasea were detected. Results The gastric mucosal was damaged seriously after 6h of WIRS in Group WC, but was damaged slightly in Group PP, SP, and SPP. Compared to those in Group NC, GI in Group WC was significantly increased, pH value and dry and moist ratio were decreased, the activity of H+-K+-ATPasea was increased (P 〈 0.01 ). Pretreatment with pantoprazole, sulfentanyl, or suientanyl and pantoprazole could decrease GI, inhibit the activity of H+-K+-ATPasea, and increase pH value, especially with pantoprazole or sufentanyl and pantoprazole (P 〈 0.01). Conclusion Sufentanil combined with pantoprazole could prevent stress acute gastric mueosal injury more effectively than sufentanil or pantoprazole alone.
