摘要
目的 研究结肠癌手术标本中 p2 7kipl表达以及在肿瘤预后中的临床意义。 方法 应用免疫组织化学SABC法检测结肠癌标本中 p2 7kipl、cyclin E、ki- 6 7的表达。 结果 结肠癌粘膜病变由良性向恶性转变 ,p2 7kipl表达的阳性率呈现逐渐降低的趋势 ;p2 7kipl的表达与结肠癌组织 Duke's分期、cyclin E及 ki- 6 7的表达呈现负相关 ,与肿瘤的预后呈正相关。 结论 结肠癌组织中 ,p2 7kipl表达缺失的肿瘤组织恶性程度更高 ,肿瘤更具有恶性增殖、浸润和转移潜能 ,并且直接影响术后生存期。检测结肠癌 p2 7kipl的表达情况 ,可以了解粘膜癌变的恶性倾向以及结肠癌的生物学行为 ,并且有助于估计患者的预后 ,指导患者术后的综合治疗。
Objective\ Expression of p27\+\{kipl\} in human colorectal carcinoma and its prognostic significance.\ This paper studied the expression of p27\+\{kipl\} in coldrectal carcinoma it prognostic significance.\ Methods\ Analyzed by immunohistochemistry the expression of p27\+\{kipl\} in a series of human tissue sample representing various stages of colorectal carcinogenesis,including 19 samples of normal colorectal mucosa,21 samples of adenoma,and 106 various types of colorectal carcinomas.\ Parallel immunostaining was done for cyclin E and also for ki-67 to evaluate cell proliferation.\ Results\ (1)Among the various colorectal specimens,the positive rates of p27\+\{kipl\} were 65 09%(69/106) in colorectal carcinoma,80 9%(17/20) in adenoma and 84 2%(16/19) in normal colorectal mucosa.\ The positive rates of p27\+\{kipl)\}kipl showed normal colorectal mucosa>adenoma>carcinoma(P<0 05).\ (2)There are inversely correlation between the degree of p27\+\{kipl\} expression with the Duke's stage of clinical pathological classification(P<0 05).\ (3)A significant,inverse correlation between p27\+\{kipl\} and cyclin E expression was observed.\ The inversely correlation between the p27\+\{kipl\} expression and the ki-67 expression could also be observed in this study(P<0 05).\ (4)In a multivariate Cox analysis,p27\+\{kipl\} expression and Duke's stage of clinical pathological classification were found to be independent prognostic factors.\ Survival analysis was performed in 106 patients.\ The 5-year survival rate of positive staining group of p27\+\{kipl\} was at 63 77%,whereas the negative staining group showed at 27 03%.\ That p27\+\{kipl\} was of negative correlation with the prognosis of colorectal carcinoma.\ Lack of p27\+\{kipl\} was associated with poor prognosis.\ By Log-rank test showed P<0 05.\ Conclusion\ The data indicated that reduction of the CKI p27\+\{kipl\} expression results in tumor cell escape from inhibition of the G\-1 to S transition of cell cycle.\ The tumor cell increases the ability of invasion and metastasis.\ The absence of p27\+\{kipl\} protein expression is thus a powerful negative prognostic marker in colorectal carcinomas,and thereby may help in the selection of patients who will benefit from adjuvant therapy.\;
