淋巴和单核细胞VCS参数在自身免疫性疾病中的变化及意义 被引量:1

摘要 目的:探讨常见自身免疫性疾病患者淋巴细胞与单核细胞体积、浆核复杂性的变化情况及其临床意义。方法:利用Beckman-Coulter Gen.S全自动血细胞分析仪的体积测量、高频传导、激光散射(VCS)技术,测定系统性红斑狼疮(SLE组)、类风湿性关节炎(RA组)和干燥综合征(SS组)患者及健康对照(正常对照组)各39例的淋巴细胞体积(LYV)、浆核比(LYC)、激光散射强度(LYS)及它们各自的平均分布宽度(SD),以及单核细胞体积(MOV)、浆核比(MOC)、激光散射强度(MOS)及它们各自的平均分布宽度(SD),并采用ROC曲线分别评价各参数的敏感度和特异度。结果:与正常对照组比较,SLE组、RA组及SS组大部分参数均有不同程度的升高(P<0.05)。其中SS组的MOV、LYC-SD在3组疾病中增高的幅度最明显,分别为172.4±8.9、13.5±2.3,高于正常对照组(163.3±5.0,10.5±1.7)、SLE组(167.3±6.9,12.2±2.7)及RA组(165.5±15.5,11.6±1.4)(P<0.05),SLE组的LYS-SD为20.0±2.6,高于正常对照组的17.8±1.6、RA组的16.8±1.8及SS组的18.2±3.5(P<0.05)。LYS-SD、MOS用于区分SLE与SS的ROC曲线下面积分别为0.656、0.625;LYS-SD、MOS-SD用于区分SLE与RA的ROC曲线下面积分别为0.836、0.630;MOV、MOS用于区分SS与RA的ROC曲线下面积分别为0.666、0.603。结论:淋巴细胞与单核细胞的VCS各参数在自身免疫性疾病患者会发生一定程度的变化,可作为诊断的辅助指标。LYS-SD、MOS、MOS-SD、MOV等可作为不同自身免疫性疾病鉴别诊断的辅助指标。
