Logistic Regression Analysis of Factors Influencing Initial Newborn Hearing Screening in Local Areas

摘要 目的探讨基层医院新生儿TEOAE听力初筛未通过的影响因素。方法对河南省安阳市妇幼保健院2008年12月到2010年3月出生、并于一周内接受TEOAE听力筛查的9 686例新生儿的资料进行回顾性分析,记录新生儿母亲孕龄、孕周(足月、早产)、分娩方式(顺产、剖宫产)以及新生儿性别、胎数(单胎、多胎)、出生体重(低体重、正常体重、巨大儿)、入住病房类别(普通病房、NICU)和筛查日龄(以0.5天累计)等临床资料,采用多因素Logistic回归分析方法,筛选出新生儿听力初筛未通过的影响因素。结果①9 686例新生儿双耳或单耳未通过5 027例(51.90%),右耳未通过3 690例(38.10%),左耳未通过4 048例(41.79%)。②新生儿性别、胎数、病房类别以及筛查日龄对筛查结果有影响,其中低日龄新生儿比大日龄新生儿通过率低(OR=0.499,P<0.001),男婴比女婴通过率低(OR=1.293,P<0.001),多胎比单胎新生儿通过率低(OR=1.397,P=0.010),NICU新生儿比普通病房通过率低(OR=1.530,P=0.008);母亲孕龄(P=0.505)、孕周(P=0.202)、分娩方式(P=0.594)以及新生儿出生体重(P=0.151)对筛查结果无影响。结论影响基层医院新生儿TEOAE听力初筛最主要的因素是筛查日龄,随着日龄增加通过率明显提高;另外,男婴、多胎和入住NICU者听力初筛通过率较低;母亲孕龄、孕周、分娩方式和新生儿出生体重对听力初筛无明显影响。 Objective To discuss factors influencing initial newborn hearing screening in local areas. Methods Selected were 9 686 newborns at Anyang Maternal and Child Health Hospital from December 2008 to March 2010. Transient evoked otoacoustic emissions(TEOAEs) were used to screen the hearing of all infants within 7 days after birth. A step--forward logistic regression analysis was used to filter out factors influencing the hearing screen- ing results from the eight variables: age of the mother, gestational weeks (full--term or preterm), mode of delivery (natural or cesarean), gender, twins or not, birth weight(low weight , normal weight or huge children), the ward category (general ward or NICU) and the screening days(0.5 days cumulative). Results (1) 5 027 cases of the 9 686 (51.90%) newborns did not pass in the hearing screening, and the fail rate of the right ear was 38.10% and 41.79% for the left ear. (2)Gender, twins, ward category, as well as screening days were statistically significant in the hear- ing screening results. The younger the babies were, the more difficult for them to pass the hearing screening (OR= 0. 499, P=0. 000) the babies in NICU were more difficult to pass the hearing screening than the general ward(OR = 1. 293, P〈0. 000) boys were more difficult to pass the hearing screening than girls(OR = 1. 397, P = 0. 010) twins were more difficult to pass the hearing screening than others(OR 1. 530, P= 0. 008) mother's age (P--0. 505), gestational weeks (P=0. 202), mode of delivery (P=0. 594), and birth weight (P=0. 151) did not show the impact on the screening results. Oonclusion The most important factor affecting the newborn hearing screening using TEOAE was screening age. As age increased the pass rate improved significantly. The pass rate of twins, boys, and babies in NICU were lower than others mother's age, gestational weeks, mode of delivery and neonatal birth weight shousd no significant effect on hearing screening.
