摘要
目的:探讨邻面去釉正畸矫治牙过敏症、邻面龋并发症发生率。方法:对邻面去釉正畸矫治的120例病例进行1～5年的追综观察,并与同期矫治的120例非邻面去釉病例作对照,统计矫治期间和矫治后患者牙齿过敏和邻面新发龋齿,并进行统计学分析。结果:邻面去釉组牙过敏症发生率为59.17%,症状大多在3个月内消失,少数症状可长达1年;对照组无1例有超过2周牙过敏症。2组比较有非常显著性差异(P<0.01)。邻面去釉组去釉邻面新龋发生率为20.83%,对照组邻面龋发病率为4.16%,2组比较有非常显著性差异(P<0.01)。邻面去釉组发生龋齿主要为"临界病例",且随时间延长有增多的趋势。结论:邻面去釉可增加牙齿过敏和龋齿并发症,特别是牙列拥挤间隙缺少5.0mm以上的"临界病例"更易发生并发症,临床应控制去釉厚度,注意抛光成形,局部进行防龋再矿化处理来消除牙齿过敏和龋齿并发症的发生。
Objective： To find out the complication rate of dentine hypersensitivity and proximal caries when inter- proximal enamel reduction treatment is applied. Methods： A tracing investigation has been carried out on 120 cases that have received interproximal enamel reduction treatment for 1 to 5 years. By contrast with 120 cases that have not received this particular treatment in the corresponding period, a statistic analysis is conducted afterwards in or- der to find out whether they suffer from dentine hypersensitivity and proximal caries both during and after the reduc- tion treatment. Results： In this study, the complication rate of dentine hypersensitivity reaches 59.17%. For most patients, this kind of symptom disappears in 3 months, but it may also last for 1 year. When it comes to the control group, however, the symptom of dentine hypersensitivity doesn＇t exceed two weeks （P〈0.01）. The complication rate of proximal caries for the experimental group is 20. 83% and the rate for the control group is only 4. 16%, which also reflects great difference. Most cases having the problem of proximal caries belong to borderline cases and the number keeps increasing. Conclusion： Interproximal enamel reduction treatment increases the chances of dentine hypersensitivity and proximal caries, especially for those borderline cases whose dental crowding space is less than 5.0mm. Therefore it is suggested that special attention should be given to the thickness of interproximal enamel re- duction.
