摘要
目的 研究白细胞滤过器 (LDF)在手术中消除心肌再灌注损伤的作用 ,探讨LDF在体外循环 (CPB)中 ,心肌保护的效果。方法 在猫CPB模型 (2 0只 )基础上应用LDF ,LDF的预充量为(10 .1± 1.7)ml,滤过速度为 (4 .3± 1.2 )ml/min。采用电子自旋共振 (ESR)法及硫代巴比妥酸(TBA)法 ,测定心肌氧自由基产量的变化 ,以及心肌组织丙二醛 (MDA)的变化。结果 滤过后白细胞滤除率为 (0 .93± 0 .0 5 ) % ,实验组较对照组氧自由基产量明显降低 (0 .2 0 8± 0 .0 13) %、(0 .0 81±0 .0 0 7) % (P <0 .0 1) ,MDA的变化为实验组 (0 .5 2 2± 0 .0 32 ) μmol/g蛋白 ,对照组为 (1.72 5± 0 .0 76 )μmol/g蛋白 (P <0 .0 1)。同时心肌超微结构显示实验组的改变明显轻于对照组。 结论 应用LDF能明显减低心肌再灌注损伤 ,起到了较好的心肌保护效果。
Objective\ To elucidate the protective effect of leukocyte depletion (LDF) on the myocardium during the heart operation with cardiopulmonary bypass (CPB) and investigate the myocardial protective effects of LDF during CPB. Methods\ In the model of CPB in cat, priming volume of LDF is (10.1±1.7) ml, and speed is (4.3±1.2) ml. By using the method of ESR and thiobarbituric acid methods, the changes of the oxygen free radical in myocardium and MDA in myocardium tissue were measured. Results\ The filtration efficiency rate for white blood cells were (0.93± 0.05 ) % ( P <0.01). The production of the oxygen free radical levels were lower in the experimental group than in the control group \[( 0.208 ±0.013) % vs (0.081±0.007) %,( P <0.01)] and the production of MDA levels were lower in the experimental group than in the control group [(0.522±0.032) nmol/mg pro vs (1.725±0.076) nmol/mg pro( P <0.01)]. Electromicroscopic studies demonstrated that the changes of myocardial ultrastructure were obviously milder in the erperimental group than in the control group. [WTHZ]Conclusion\ LDF can significantly attenuate myocardial reperfusion injury and have a protective effect on the myocardium.\;
