摘要
目的 动态观察准分子激光屈光性角膜切削术 (PRK)后角膜修复过程中的组织形态改变。方法 2 4只兔右眼按近视 -10 0 0D行PRK术。按术后即刻、2 4小时、1、2周、1、3、6月及 12月 8个时间点观察 ,每个时间点 3只。裂隙灯显微镜检查各组角膜上皮修复及角膜雾状浑浊 (haze)情况 ,并取角膜行光镜及电镜观察。结果 PRK术后早期角膜上皮易剥脱 ,haze的发生率为 10 0 %。PRK术后角膜上皮增生 ;新合成的基底膜不连续 ,半桥粒密度稀疏 ;角膜切削区基质浅层成纤维细胞数量增多并被激活 ;角膜基质浅层的胶原纤维排列紊乱长达 12月。结论 PRK术后早期角膜上皮的易脱落是由于PRK术对角膜上皮粘附结构的破坏所致 ;haze的出现与角膜组织的修复过程密切相关。
To investigate the morphological change of corneal tissue repairing after photorefractive keratectomy (PRK).Methods 24 adult New Zealand white rabbit received PRK in the right eye for -10.00 D ablation.At once,24 hour,1 week,2 week,1 month,3 month,6 month and 12 month after surgery,the corneas were examined periodically by slip lamp microscopy,and the right eyes of 3 rabbits were enucleated ,the corneas were studies with light microscopy and transmission electron microscopy.Results By 1 week,2 eyes had epithelial defect (8.3%) after the surgery;by 1 month,4 eyes had epithelial erosion (16.7%).All corneas developed some subepithelial haze (100%).A slightly haze appeared in the ablated area 1~2 week.The haze was most dense 1 month after PRK.Then haze faded,and almost disappear 12 month after PRK.The morphological change of corneal tissue repairing process after PRK were:Epithelium was hyperplasia after recovering.The newly synthesized basement membrane was discontinuous.The density of hemidesmosome was decreased.The number of firbroblasts in the ablated stroma were increased and actvated.The diameter of fibrils and the space between the fibrils were irregular in the anterior stroma.The collagen fibrils were disorganized and last for 12 month. Conclusions Early and repeated epithelial erosion with persistent epithelial defects might occur because of the destruction of epithelial adhesion structure after PRK.The formation of haze had close relationship with the process of corneal tissue repairing.
