摘要
目的:探讨检测新疆维吾尔族慢性肺源性心脏病(CPHD)患者血浆B型脑钠肽(BNP)水平的临床意义。方法:2011年1月~2012年6月在我种住院治疗维吾尔族CPHD患者51例,根据纽约心脏病协会(NYHA)分级标准分为心功能Ⅰ级组(n=16)、心功能Ⅱ级组(n=14)、心功能Ⅲ-Ⅳ级组(n=21),以单纯COPD维吾尔族患者15例为对照。用免疫荧光法检测血浆BNP浓度,超声心动图检测右室舒张末期内径(RVD)、肺动脉舒张期末期内径(PAD)、肺动脉收缩压(PAP)等。结果:随着NYHA分级的增加,血浆BNP水平随之上升,其中NYHA心功能Ⅱ-Ⅳ级患者BNP水平明显高于心功能I级或单纯COPD患者(P<0.05)。并且维吾尔族CPHD,患者BNP水平与RVD(r=0.293,P=0.017)、PAD(r=0.258,P=0.036)、PAP(r=0.410,P=0.001)成:正相关。结论:维吾尔族CPHD患者血浆BNP水平与心衰程度、心超检测结果等具有较好的相关,可以作为少数民族地区比较简便及准确的评估、随访病情的指标之一。
Objective To investigate the changes of the levels of plasma brain natriuretic peptide (BNP) in uyghur patients with chronic pulmonary heart disease (CPHD) in Xinjiang. Methods Plasma BNP levels were detected in 51 uyghur patients with CPHD who adiministrated into our hospital from 2011 Jan to 2012 Jun, and compared with those of simple COPDuyghur patients (n=15). CPHD patients were divided to NYHA I (n=16), NYHA II (n=14) and NY- HA III-IV (n=21) groups. BNP was measured by immunofluorescence and Cardiac function was evaluated by ultra- sonography. Results the BNP level increased with the NYHA class, NYHA II-IV groups had significantly higher BNP levels than simple COPD or NYHA I group (P 〈0.05). Uyghur CPHD patients BNP levels are positively correlated with right ventricular diameter (r=0.293, P=0. 017), pulmonary artery diameter (r = 0.258, P=0. 036) and pulmonary artery pressure (r=-0.410, P=0.001). Conclusions BNP level may indicate the symptom and the degree of cardiac dys- function in uyghur CPHD patients, it can be used to help clinical evaluation and treatment of uyghur CPHD.
