摘要
目的探讨清解活络汤治疗儿童髋关节滑膜炎(TSH)的临床效果。方法将65例患儿随机分为中医组33例,西医组32例,中医组采用自拟清解活络汤水煎,取药汁200mL,1剂/d,分2次或少量多次温服,再用以上药渣加水煎至沸后先用热气熏蒸患处,待水温适可后用药渣或药液浸泡患处,每日2次;西医组采用利巴韦林氯化钠注射液15～20mg/(kg·d)1次静脉滴注,甲钴胺注射液500μg肌内注射,隔日1次;布洛芬口服液4～8mL/次,疼痛明显时服用。两组治疗时间均7d为1个疗程,连续治疗2周。治疗期间严格要求患儿卧床休息。随访1年。结果中医组痊愈率(60.61%)与总有效率(97.97%)均明显高于西医组(P<0.01)。随访1年,西医组复发4例,中医组无复发(χ2=25.94,P<0.05)。结论清解活络汤治疗儿童TSH临床疗效明显,且安全无复发。
Objective To investigate the clinical curative effect of qingjiehuoluo decoction in treatment of transient synovitis of the hip fiSH）. Methods Sixty five cases were randomly divided into traditional Chinese medicine （TCM） group （33 cases） and Western medcine group （32 cases）. TCM group were given qingjiehuoluo decoction. Qingjiehuoluo decoction was boiled into 200 mL decoction, 1 decoction per day, take warm decoction in twice or more times. Then the medicine remainder was boiled with water and fumigated afflict hip with the hot steam, and bathed afflict hip with warm remainder or deeotetion, Bid. Western medicine group were given ribavirin and sodium chloride injection [15-20 mg/（kg.d）, ivgtt, qd] and mecobalamin injection （500 p.g, im, Qod）. Bloven oral solution （4-8 mL） was given when child pain seriously. Both were given 7 days as a curative course, and treated 2 weeks. During therapy period, patients must stay in bed. All patients were followed for 1 year. Results Cure rate of TCM group was 60.61% and effective rate was 97.97%, which was better than Western medicine group （P 〈 0.01）. All patients were followed up for 1 year, 4 cases were recurrence in Westem medicine group, and none in TCM group （χ2 =25.94, P 〈 0.05）. Conclusion The cu- rative effect of qingjiehuoluo decoction in treatment of TSH is obvious. It is safe and no recurrence.
