前列腺尿道部组织射频消融中冷却法保护尿道的实验研究

摘要 目的:通过冷却法对尿道保护的研究,以指导临床提供消融过程中消融针距离尿道的安全距离。方法:4只犬两侧前列腺共消融了11个灶,三通Foley导尿管插入膀胱可以持续注入冰水混合的生理盐水,经会阴部超声监视下进针,测量子针针尖距离尿道的距离,在消融过程中应用超声造影剂进行实时监控消融范围。结果:组1(n=4)无尿道冷却保护;组2(n=6)进行尿道冷却保护,组1和2的10个消融灶子针针尖距离尿道的距离≥0 cm,进行尿道损伤情况的fisher确切检验法P=0.005。组3为1个消融灶,子针针尖伸入尿道壁。测量11个灶病理标本(56.8±33.9)mm2和超声造影下(68.7±25.0)mm2消融灶的前后径和左右径,进行乘积后进行配对t检验,检验结果显示两种方法有很好的相关性(r=0.805,P=0.005),在评估消融灶大小方面经t检验差异无统计学意义(t=1.86,P=0.096)。结论:在前列腺尿道部组织的消融中冷却法保护尿道安全距离是子针针尖距离尿道≥0 cm。在消融过程可应用造影剂监测消融范围,便于更加直观地显示消融灶与尿道的关系。 Objective:To evaluate the safe distance between the tip and urethra can protect the urethra by using cooling on radiofrequency ablation of prostate.Methods:11 thermal lesions were produced on both sides of the prostate in 4 dogs.Three-way Foley catheters(14 French) will be inserted into the bladder and used to allow continuous infusion with cold saline.To monitor needle localization by transperinea ultrasound and measure the distance between needle tip and urethra.With the use of contrast-enhanced imaging,we can observe the size of thermal lesions.Results:Four adult male dogs were divided into three groups.Group 1(n=4),a control group without additional protective cooling.Group 2(n=6),with protective cooling of the urethra.The distance of all the ten lesions between the tip and urethra is more than or equal to 0cm.Damage to the urethra was assessed independently in 4 quadrants using fisher exact test(P=0.005).The tip was inserted into urethra in one lesions of group 3.There was good correlation between the lesion sizes measured with multiplied data of the antero-posterior and transverse lengths as determined with contrast-enhanced imaging and pathological measurements,there is no significant differences in T-test method.Conclusion:The distance between the tip and urethra is equal to or 0cm can protect the urethra by using cooling.Contrast imaging dramatically improved the visualization of the thermal RF lesions and urethra.
