摘要
目的探讨肝炎后肝硬化患者Tei指数及其相关参数的变化与肝功能Child-Pugh分级的关系。方法选择研究对象共197例,其中正常对照组60例、慢性肝炎组68例,肝炎后肝硬化组69例。肝炎后肝硬化组69例患者,按照Child-Pugh分级标准又分为Child A级20例,Child B级24例,Child C级25例。采用彩色超声诊断仪测量并比较各组左心室Tei指数及相关参数的差异,以及其与肝功能Child-Pugh分级的关系。结果 (1)肝炎后肝硬化组与慢性肝炎组相比,Tei指数、IRT/ET参数明显增高(P<0.05);但ICT/ET参数,两组无明显差异(P>0.05)。(2)Child A级、Child B级、Child C级患者Tei指数、IRT/ET参数呈逐渐增高趋势,3级之间存在显著性差异(P<0.05);而ICT/ET参数,3级之间无统计学差异(P>0.05)。结论肝炎后肝硬化患者肝脏储备功能损害越严重,Child-Pugh分级评分越高,Tei指数就越高,心脏功能也越差,且以左心室舒张功能受损为主。
Objective To discuss the correlation between Tei index and correlative parameters and the Child - Push grading of hepatic function in patients with posthepatitic cirrhosis. Methods 197 subjects were selected with 60 ones in normal/control group,68 ones in chronic hepatitis group, and 69 ones in posthepatitic cirrhosis group. Those in the posthepatitic cirrhosis group were divided intre grade Child A(20 cases) ,grade Child B(24 cases) ,and grade Child C(25 cases)according to the Child - Pugh grading standard. Color uhrasonograph was used to detect the left ventricle Tei indexes of each group. Comparison was made in the differences of the parameters among all groups and the correlation between those indexes and the Child - Pugh grading of hepatic function. Results ( 1 ) The Tei index and IRT/ET parameter of the posthepatitic cirrhosis group significantly increased compared with those of the chronic hepatitis group( P 〈 0.05), but there was no significant difference in the ICT/ET parameter between the two groups (P 〉 0.05 ). (2)The Tei index and IRT/ET parameter had a tendency of increasing in patients of grade Child A, B, and C, and the differences were significant among the three grades(P 〈 0.05). But the difference in ICT/ET parameter was not significant among the patients of the three grades (P 〉 0.05). Conclusion The more severe the hepatic function lesion in patients with posthepatitic cirrhosis, the higher the scores of Child - Pugh grading and the Tei index, and the worse the cardiac function especially the diastolic function lesion of left ventricle.
