老年冠心病患者脂蛋白(a)与纤维蛋白原的相关性分析 被引量:9

摘要 目的探讨脂蛋白(a)[Lp(a)]和纤维蛋白原(Fib)在老年冠状动脉粥样硬化性心脏病(冠心病)发病中的作用以及二者相互关系。方法选择784例65岁以上老年患者,其中对照组(非心脏病)116例,冠心病组668例(其中1支病变组106例,2支病变组120例,3支病变组442例),分别测定其Lp(a)和Fib的水平,并按分组及性别分别进行比较分析。结果(1)男性组随着冠脉病变的逐渐加重,Lp(a)水平亦逐渐升高,对照组、1支组、2支组、3支组分别为(0.22±0.16)、(0.28±0.23)、(0.32±0.19)、(0.34±0.21)g/L,差异有统计学意义(F=9.22,P〈0.01),女性组则未见此规律。(2)无论男性和女性,随着冠脉病变的逐渐加重,Fib水平均逐渐升高,男性组分别为(3.24±0.82)、(3.414±0.82)、(3.454±0.89)、(3.614±0.90)g/L;女性组分别为(3.58±0.97)、(3.69±1.20)、(3.77±O.88)、(4.09±1.05)g/L,差异有统计学意义(F=3.09,P〈0.05;F=4.34,P〈0.01)。(3)男性组Lp(a)与Fib呈正相关(r=0.15,P〈0.01)。(4)多元Logistic回归分析表明,Lp(a)和Fib是冠心病的影响因素(OR=0.31、1.32,均P〈0.05)。结论Lp(a)和Fib与老年男性冠心病严重程度密切相关。 Objective To explore the effects of lipoprotein(a) ( Lp(a) ) and fibrinogen (Fib) in senior patients with coronary heart disease (CHD) and examine the relationship of Lp(a) and Fib. Methods Based upon the results of coronary artery angiography (CAG) , 784 subjects aged over 65 years were divided into control (n = 116) group, one-vessel ( n = 106) group, two-vessel ( n = 120) and three-vessel ( n = 442) groups. And the levels of Lp (a) and Fib were measured. Results ( 1 ) With increasing number of involved branches, the level of Lp ( a ) gradually rose in male group ( ( 0. 22 ± 0. 16 ), ( 0. 28± 0. 23 ), (0.32±0. 19), (0.34±0.21) g/L; F=9.22, P〈0.01); (2) whether for males or females, with increasing number of involved branches, the levels of Fib gradually rose in male group ( ( 3.24 ± 0. 82 ), (3.41 ±0. 82), (3.45±0. 89), (3.61 ±0. 90) g/L; F =3.09, P 〈0. 05) and in female group ( (3.58± 0.97), (3.69±1.20), (3.77±0.88), (4.09±1.05) g/L; F=4.34, P〈0.01); (3) the level of Lp (a) had a positive correlation with Fib in male group (r =0. 15, P 〈0. 01 ) ; (4) multi-Logistic regression analysis showed that Lp(a) and Fib were influencing factors of CHD (OR =0. 31, 1.32, both P 〈0. 05). Conclusion There are significant correlations between Lp(a) , Fib and the severity of CHD in senior male patients.
