心肌血流储备分数在临界病变的非ST段抬高急性冠脉综合征患者中的应用 被引量:3

摘要 目的:探讨心肌血流储备分数(fractional flow reserve,FFR)在稳定临界病变的非ST段抬高急性冠脉综合征(non-ST-segment elevation acute coronary syndrome,NSTEACS)患者中的应用。方法:冠状动脉造影(coronary angiography,CAG)明确冠状动脉病变直径狭窄40%~70%的NSTEACS、FFR测定值≥0.75不需行经皮冠脉介入治疗(percutaneous transluminal coronaryintervention,PCI)的患者32例,排除CAG显示的不稳定斑块,平均年龄(68.6±11.4)岁。主要终点事件为1年内的心源性死亡、非致死性心肌梗死、靶病变血运重建及因心脏原因再入院。结果:32例患者共36处病变,14例(43.8%)为非ST段抬高心肌梗死,CAG显示的平均直径狭窄率为(41.6±7.8)%,平均FFR值为0.86±0.08,术中无并发症发生。1年随访率100%;未见死亡和致死性心肌梗死发生,1例(3.1%)靶血管血运重建和2例(6.3%)因心脏原因再入院,90.6%患者未见任一主要终点事件发生。结论:对排除不稳定斑块且CAG显示冠状动脉临界病变的NSTEACS患者,FFR测定有助于避免不必要的血运重建。
