摘要
目的探讨镍铬合金和钛合金烤瓷冠修复对牙周组织及龈沟液中细胞因子的影响。方法 80例共80颗牙按照烤瓷冠金属材料不同分为镍铬合金组(n=40,采用镍铬合金烤瓷冠修复)和钛合金组(n=40,采用钛合金烤瓷冠修复),以对侧同名健康天然牙作为对照。比较两种烤瓷冠修复后1年对牙龈的影响,检测患牙和对照牙龈沟出血指数(SBI)、菌斑指数(PLI)、探诊深度(PD);测定并比较龈沟液(GCF)中白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)、天冬氨酸转氨酶(AST)水平。结果镍铬合金组对牙龈外观的影响大于钛合金组(P<0.01);修复后1年,镍铬合金组修复牙SBI及PD均显著大于对照牙(P<0.05),PLI与对照牙无差异(P>0.05);钛合金组修复牙SBI较对照牙增大(P<0.05),PLI及PD与对照牙无差异(P>0.05);镍铬合金组修复牙GCF量较对照牙增多,IL-1β、TNF-α及AST水平均较对照牙升高(P<0.05、P<0.01),而钛合金组修复牙GCF量、IL-1β、TNF-α及AST水平较对照牙无差异(P>0.05)。结论镍铬合金烤瓷冠对牙龈和牙周组织有明显不利影响,而钛合金烤瓷冠对牙龈和牙周组织的影响较小,值得临床借鉴。
Objective To explore the effect of Ni-Cr alloy and Ti-Lite alloy porcelain fused metal crown on periodon- tal tissues and cytokines in gingival crevicular fluid. Methods A total of 80 cases with 80 teeth were divided into Ni- Cr alloy group (n=40, repaired with Ni-Cr alloy porcelain fused metal crown) and Ti-Lite alloy group (n=40, Ti-Lite alloy porcelain fused metal crown), with healthy teeth as controls. The effect of two PFM crowns on gum after repaired for 1 year was compared, and the SBI, PLI, PD, GCF, IL-1β, TNF-α and AST of offending teeth and healthy teeth in two groups were compared. Results Ni-Cr alloy group showed more severe effect on the appearance of gum than Ti- Lite alloy group (P 〈 0.01), the repaired teeth had higher SBI and PD than healthy teeth after 1 year (P 〈 0.05), but no statistical difference in PLI between repaired teeth and healthy teeth was observed (P 〉 0.05); GCF amount, IL-1β, TNF-α and AST levels were higher than healthy teeth. In Ti-Lite alloy group, the repaired teeth had higher SBI than healthy teeth (P 〈 0.05), but no statistical difference in PLI, PD, GCF amount, IL-1β, TNF-α, AST levels between re- paired teeth and healthy teeth was observed (P 〉 0.05). Conclusion Ni-Cr alloy porcelain fused metal crown has obvi- ously adverse effects, and Ti-Lite alloy porcelain fused metal crown shows a small effect on gum and periodontal tis- sues, thus Ti-Lite alloy porcelain fused metal crown is worthy of applied in clinic.
