重症急性胰腺炎大鼠甲状腺滤泡上皮细胞功能和超微结构的变化 被引量:4

Ultrastructural and functional changes of thyroid follicular cells in rats with severe acute pancreatitis

摘要 目的观察重症急性胰腺炎(SAP)时甲状腺滤泡上皮细胞超微结构和功能的变化。方法雄性Wistar大鼠40只,体质量200-250g(湖北省疾病预防控制中心提供),随机分为5组:假手术组(SO组)、重症急性胰腺炎模型1、3、6、12h组(SAP1、346、12h组)。胆胰管逆行注射5%牛磺胆酸钠制备SAP模型。测定淀粉酶(AMY)、磷脂酶A2(sPLA2)、T3、T4、游离甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平。抽血后取甲状腺腺叶组织,光镜观察病理改变,电镜下观察滤泡上皮细胞超微结构改变。结果与S0组比较,功能上SAP组大鼠血AMY、sPLA2水平明显升高,分另0为(2489±442-5253±717)U/L和(621.33±23.77-1021.11±54.17)mol/(min·m1)(P〈0.05),血T3、FT3、FT4水平明显下降,结果分别为(0.55±0.21-0.76±0.28)nmol/L、(0.93±0.15-1.98±O.63)nmol/L和(0.63±O.37-3.29±1.49)nmol/L(P〈0.05),T4水平先升高后降低,结果为(20.70±5.75-42.95±19.82)nmol/L,差异有统计学意义(P〈0.05)。形态学上,滤泡增生、融合,滤泡上皮细胞脱落,细胞核膜内陷,微绒毛减少,粗面内质网脱颗粒,高尔基体、线粒体肿胀,间质纤维增生,毛细血管扩张、充血。结论机体甲状腺激素水平的改变,可作为评估SAP病情严重程度的评估指标。 Objective To observe the ultrastructural and functional changes of the thyroid follicular cells in rats with severe acute pancreatitis (SAP)and then to provide a theoretical basis for clinical treatment of thyroid damage in patients with SAP. Methods Forty Wistar rats weighing 200-250 g were randomly divided into 5 groups : the sham operation group (SO group) and the SAP 1, 3, 6, 12 h groups (8 rats per group). SAP model was induced by retrograde injection of 5% sodium taurocholate into the biliopancreatic duct. AMY, sPLA2, T3, T4, FT3 and FT4 in serum were determined. The pathological and ultrastructural changes in the thyroid tissue were observed under the light microscopy and TEM respectively. Results As compared with SO group, the AMY, sPLA2, T3, FT3 and FT4 levels in SAP groups were significantly reduced in serum: (2489 ± 442-5253 ±717) U/L, ( 621.33 ± 23. 77-1021.11 ±54.7 ) moL/( min· ml), (0.55±0.21-0.76±0.28) nmol/L, (0.93±0.15-1.98±0.63) nmol/L'and (0.63 ±0.37-3.29± 1.49) nmol/L (P 〈 0. 05 ); The T4 levels were increased first and then decreased: (20. 70± 5.75- 42. 95 ± 19.82) nmol/L (P 〈 0. 05 ). Morphologically, thyroid follicles were increased and thyroid follicular cells exfoliated, nuclear membrane retracted, microvilli reduced, rough endoplasmic reticulum degranulated, Golgi apparatus and mitochondfia enlarged, interstitial fibrosis increased, and capillaries were expanded and filled will blood cells. Conclusion The changes of thyroid hormones can be used to evaluate the severity of SAP.
