摘要
目的探讨慢性重型乙型肝炎(chronic severe hepatitis B,CSHB)患者血清乙肝病毒复制(HBV DNA)水平与病情严重程度及预后的关系。方法选择2009年8月～2012年8月我院治疗的乙型肝炎患者78例,分为慢性乙型肝炎(CHB)组48例,CSHB组30例;应用实时免疫荧光定量PCR法检测血清HBV DNA水平,并与总胆红素(TBil)、凝血酶原活动度(PTA)、血清白蛋白(Alb)、血清胆碱酯酶(CHE)、总胆固醇(Chol)、甲胎蛋白(AFP)等生化指标进行相关性分析,比较CSHB患者存活组与死亡组HBV DNA水平。结果 CSHB组患者血清HBVDNA阳性率及其水平与CHB组比较差异无统计学意义(均P>0.05);CSHB组患者血清HBV DNA水平与TBil呈显著正相关(P﹤0.01),与PTA、Alb、CHE、Chol、AFP呈显著负相关(P﹤0.05或P﹤0.01);CSHB组中死亡组患者血清HBV DNA水平显著高于存活组,差异有统计学意义(P﹤0.05)。结论 CSHB患者血清病毒复制水平与病情严重程度及预后有明显相关性,CSHB毒复制活跃者,预后不良。
Objective To study the relationship between hepatitis virus B replication（HBV DNA） levels and disease severity and prognosis in patients with chronic severe hepatitis B（chronic severe hepatitisB,CSHB）.Methods 78 patients with hepatitis B in our hospital from August 2009 to August 2012 were selected,all the patients were divided into chronic hepatitis B（CHB） group with 48 cases and CSHB group with 30 cases;real-time immunofluorescence quantitative PCR was used to detected the serum HBV DNA levels,and the correlation was analyzed between serum HBV DNA levels and biochemical indicators such as Tbil,PTA,Alb,CHE,Chol,AFP;the HBV DNA levels were compared in survived groups and death groups in CSHB patients.Results The level of serum HBV DNA in death group was higher than that in survival group,the difference was statistically significant（P ﹤ 0.05）.The difference of serum HBV DNA positive rates and levels between CSHB group and CHB group were not statistically significant（P〈0.05）.The positive correlation was found between the levels of serum HBV DNA and the levels of Tbil（P ﹤ 0.01）,the negative correlations were found between the levels of serum HBV DNA and the levels of PTA,Alb,CHE,Chol and AFP（P ﹤ 0.05 or P ﹤ 0.01）.Conclusion The serum levels of viral replication has significantly positive correlation with disease severity and prognosis and the patients with active virus replication have poor prognosis.
