摘要
目的回顾社会支持与老年人抑郁关系的调查研究，探讨二者之间的关系。方法采用计算机检索和手工检索相结合的方法搜索PubMed、CNKI、万方等数据库中关于社会支持与老年人抑郁关系的病例对照研究和横断面研究，检索日期从建库至2012年8月1日。应用纽卡斯尔-渥太华量表（The Newcastle—Ottawa Scale，NOS）对符合纳入标准的文献进行质量评价和资料提取，采用Review5．1．1．0统计学软件进行Meta分析。结果共纳入11篇文献（共1722例）。抑郁组老年人与非抑郁组老年人相比，客观支持方面无统计学意义[SMD=-0．26，95％CI（-0．61，0．08），P=0．13]，主观支持[SMD=-0．74，95％CI（-1．04，-0．44），P〈0．00001]，社会支持利用度[SMD=-0．67，95％CI（-0．99，-0．35），P〈0．0001]和社会支持总分[SMD=-1．02，95％CI（-1．43，-0．62），P〈0．00001]方面有统计学意义。结论主观支持和社会支持利用度较低是老年人抑郁的危险因素。
Objective This study was to review investigations and studies on the correlation between social sup- port and elderly depression, and to discuss the correlation between social support and elderly depression. Methods PubMed, CNKI,Wanfang were searched for papers of the correlation between social support and elderly depression from the time database was built to August 1,2012. The quality of researches was critically appraised and data were extracted by 2 investigators independently. Meta analysis was used to analyze 11 literatures（ 11 cases total）correspond- ing to the standard. Results Comparison was made between old people with and without depression. The combined mean difference of objective support was - 0.26 [ 95 % CI （ - 0.61 - 0.08 ）, P = 0.13 ] ; the combined mean differ- ence of subjective support was - 0.74 [ 95% CI（ - 1.04 - - 0.44） , P 〈 0. 00001 ] ; the combined mean difference of social support utilization was - 0.67 [ 95 % CI （ - 0.99 - - 0. 35 ）, P 〈 0.0001 ] ; the combined mean difference of social support total fraction was - 1.02 [ 95 % CI （ - 1.43 - - 0.62 ）, P 〈 0. 00001 ]. Conclusion Short of sub- jective support and low social support utilization were risk factors of elderly depression.
