摘要
目的 探讨风湿性心脏病心肌组织 , 型前胶原m RNA的表达和定位 ,以及胶原改建对心功能的影响 .方法 用人 , 型前胶原 c DNA探针 ,对 32例风湿性二尖瓣乳头肌进行分子原位杂交并进行图像分析 .结果 病变心肌间质中 , 型前胶原 m RNA阳性表达明显增多 , 型较正常成年心肌中增高 1.6 7倍 , 型增高 4.36倍 ;病变心肌细胞核内出现 , 型前胶原 m RNA阳性表达 ,胎儿心肌亦有类似表达 ; , 型前胶原 m RNA阳性表达量与左室射血分数 ( EF) ,左室周径向心缩短率 ( m Vcf)呈负相关 .结论 风心病心肌细胞参与了胶原的合成和分泌 ,并对心脏功能有严重影响 ;风心病心肌胶原增生重现了胎儿期合成胶原的表型 .
AIM To explore the expression and localization of typeⅠ and Ⅲ precollagen mRNA in rheumatic myocardium and the effect of collagen remodeling on heart function. METHODS Hybridization in situ and imaging analysis were performed with type Ⅰ, Ⅲ human precollagen cDNA probes in 32 rheumatic mitral papillary muscles. RESULTS Positive expression of typeⅠ, Ⅲ precollagen mRNA in rheumatic myocardial tissue increased significantly; TypeⅠ increased to 1.67 times and type Ⅲ increased to 4.36 times more than that in the normal adult myocardium. The positive expression of type Ⅰ, Ⅲ precollagen mRNA appeared in the nucleus of rheumatic hypertrophic cardiomyocytes. The similar positive expression also appeared in fetal myocardium. The degree of positive expression of type precollagen mRNA had a negative relationship with LVEF and mVcf. CONCLUSION Rhaumatic cardiomyocytes take part in the collagen synthesis and secretion, which significantly affect the heart function. The collagen hyperplasia in rheumatic myocardium revealed the phenotype of collagen synthesis during the period of fetus. This suggested that the prevention and treatment of chronic rheumatic heart disease with regulation and degression of the collagen hyperplasia in myocardium may be possible.
