摘要
目的 观察高压氧 (HBO)治疗交通事故造成肢体创伤的康复治疗效果。方法 对交通事故造成的 486例肢体创伤 (骨折、断指创伤后植皮、皮瓣转移 )加用HBO治疗 ,并以常规治疗的2 5 6例作为对照。测定HBO治疗前后患者血浆中的中分子物质含量。并进行了小鼠创口HBO治疗愈合的实验。结果 HBO治疗组在肢体创伤后骨折愈合、断指 (肢 )再植及植皮、皮瓣转移成活率明显提高 ,χ2 =7.5 6 ,11.45 ,35 .17,均P <0 .0 1。小鼠创口愈合 ,HBO治疗组亦明显优于对照组。患者HBO治疗后血浆中的中分子物质含量下降 ,从 ( 412 0± 310 )U/L下降至 ( 2 46 0± 330 )U/L。 结论HBO治疗可以促进肢体骨折的愈合 ,提高断指 (肢 )再植和植皮、皮瓣转移的成活率。血浆中的中分子物质与创伤程度有密切相关。
Objective To explore the effect of hyperbaric oxygenation(HBO) on extremity wound caused by traffic accident.Methods 486 cases with extremity wound including fracture,skin replantation after digit-severed and flap transfer,were treated with HBO comprehensive treatment,another 256 cases were only treated with conventional treatment as control group.The content of plasma medium molecule material was measured at pre- and post-HBO treatment.At the same time,animal experiment was carried out in ten mice,which were also treated with HBO.After HBO treament,the healing of wound was observed.Results The healing of fracture,survival rate in skin replantation after digit-severed and flap transfer were significantly raised(χ 2=7.56,11.45,35.17,P<0.01).On animal experiment,the healing of wound in mice of HBO group was superior to the control group.While the content of medium molecule material was reduced,which was from (4 120±310)U/L down to (2 460±330)U/L.Conclusion It is showed that HBO treatment may promote the healing of fracture,raise the survival rate of skin replantation and flap transfer.It also may reduce the content of medium molecule material,which is related to wound healing.
