摘要
目的分析丹东地区2型糖尿病(T2DM)患者骨质疏松症(OP)情况及骨质疏松症相关危险因素。方法测定232例T2DM患者腰椎及股骨颈(Neck)骨密度(BMD),根据1998年WHO制定的OP诊断标准把T2DM患者分为OP组与非OP组,比较两组生化指标、BMD差异,并进行相关性分析。结果 1.T2DM患者中OP组与非OP组之间生化资料比较示OP组低密度脂蛋白(LDL-C)较非OP组明显高,存在显著性差异(P<0.05)。2.OP组与非OP组腰椎(L1-4)BMD比较示两组之间无显著性差异(P>0.05);OP组的总体、男性及女性的平均股骨颈(Neck)BMD明显低于非OP组,两组之间存在显著性差异(P<0.05)。3.T2DM患者中OP组65例,总体发病率28.0%(65/232),其中男性8例,男性发病率12.5%(8/64);女性57例,女性发病率33.9%(57/168),男女OP发病率比较,存在显著性差异(χ2=10.552,P<0.05)。4.Logistic回归分析示:T2DM患者中女性、有微血管并发症(糖尿病肾病或/和糖尿病视网膜病变)、有高血压、血清钙离子低、低体重指数与OP发生相关(P<0.05)。结论 1.丹东地区住院T2DM患者的OP发病率28.0%,明显高于正常人群,且女性发病率高于男性。2.丹东地区T2DM患者中女性、有微血管并发症(糖尿病肾病或/和糖尿病视网膜病变)、有高血压、血清钙离子低、低体重指数与OP发生相关。
Objective To investigate the incidence of osteoporosis (OP) in patients with type 2 diabetes (T2DM) , and to analyze the risk factors of OP in Dandong area. Methods Bone mineral density (BMD) of the lumbar vertebrae and the femoral neck in 232 patients with T2DM was tested. All the patients were divided into two groups: OP Group and non-OP Group, according to the 1998 WHO standard of OP diagnosis. Biochemical index and BMD of patients in both groups were compared and analyzed for correlation. Results The comparison of biochemical index of T2DM patients in both OP Group and non-OP Group showed that LDL-C in OP Group was higher than that in non-OP Group, and the difference was significant (P 〈 0. 05 ). BMD of the lumbar vertebrae in both groups had no significant difference (P 〉 0.05). The average BMD of the femoral neck of all male and female patients in OP Group was significantly lower than that in non-OP Group ( P 〈 0. 05 ). The OP Group had 65 patients, and the incidence of OP in all T2DM patients was 28.0%. Among all these patients, 8 were male with an incidence of 12.5% (8/64) , and 57 were female with an incidence of 33.9% (57/168). The incidence of OP in females and males had significant difference (χ2 = 10. 552, P 〈 0.05). Logistic regression analysis showed that female gender, micro-vascular complications (DN or DR) , hypertension, low serum calcium, and low BMI were correlatedwith the genesis of OP in T2DM patients. Conclusion The incidence of OP in hospitalized T2DM patients in Dandong area is 28.0% , which is much higher than normal population. And the incidence of osteoporosis in women is higher than that in men. Risk factors including female gender, micro-vascular complications (DN or DR), hypertension, low serum calcium, and low BMI are correlated with the genesis of OP in T2DM patients in Dandong area.
