摘要
目的分析肺癌患者医院感染的特点与危险因素,以制定相应预防和护理对策。方法采用病例对照研究模式,回顾性分析128例肺癌患者临床资料,对可能的影响因素进行单因素分析及多元回归分析,确定影响肺癌患者医院感染的独立危险因素。结果 128例肺癌中20例发生医院感染,医院感染为15.6%;20例医院感染中,12例为呼吸道感染,5例胃肠道感染,3例泌尿道感染;统计分析结果显示,临床分期、使用三代头孢、侵入性操作、放化疗和住院时间是肺癌患者发生医院感染的相关因素(P<0.05)。结论肺癌患者是医院感染的高危人群,医院感染是多种因素共同作用的结果,控制感染的危险因素、规范抗肿瘤治疗、提高机体免疫力是预防控制肺癌患者医院感染的主要措施;应对护理人员进行相关知识和能力的培训,提高控制医院感染意识,以达到预防医院感染的目的。
OBJECTIVE To study the related factors :;or nosocomial infections in patients with lung cancer so as to propose corresponding nursing measures. METHODS By means of case-control study, the medical records of 128 patients with lung cancer were retrospectively analyzed. Univaiate analysis and multiple regression analysis were performed to analyze the related factors for nosocomial infections so as to determine the independent risk factors for the nosocomial infections in the lung cancer patients. RESULTS Of 128 patients with lung cancer, the nosocomial infections occurred in 20 cases with the infection rate of 15.6%. Of the 20 patients with nosocomial infections, there were 5 patients with gastrointestinal tract infections and 3 cases with the urinary tract infections. The study indicated through the statistical analysis that the clinical stage, invasive operation, chemotherapy, use of the third generation of cephalosporins, and hospital stay were the related factors for the nosocomiai infections in the lung cancer patients. CONCLUSION The lung cancer patients are at high risk of nosocomial infections, the incidence of nosocomial infections results from the joint effect of multiple factors. It is an effective way to prevent the nosocomial infections by controlling the risk factors, standardizing the anti-tumor therapy,and improving the immunity. It is necessary for the nursing staff to accept training, conform the disinfection regulations, improve the awareness of nosocomial infections control so as to prevent the nosocomiai infections.
