摘要
目的采用Meta分析方法系统评价检测血浆K-ras基因第12密码子点突变对胰腺癌早期诊断的价值。方法由两名统计人员电子检索PubMed数据库、EMbase数据库、中国生物医学文献数据库、中国知网,文献检索起止时间均为建库至2012年5月31日。按照纳入与排除标准筛选文献,同时逐篇进行质量评价和诊断准确度的资料提取。应用Meta-DiSc1.4软件进行Meta分析,Stata12.0软件进行发表偏倚评价。结果共纳入10篇文献,包括690例研究对象。异质性检验无阈值效应,但是存在其他异质性。对PCR-RFLP亚组进行分析,敏感性为0.714(95%CI:0.649-0.773),特异性为0.868(95%CI:0.781-0.930),SROC中AUC=0.8544,Q*=0.7853。结论现有资料表明检测血浆K-ras基因突变对胰腺癌早期诊断效能中等,单独作为胰腺癌早期诊断指标尚有不足,可以作为胰腺癌早期诊断检测方法的重要补充,是胰腺癌早期诊断的可选检查方法。
Objective To systematically assess the diagnostic value of detection of K-ras gene mutation at codon 12 in pancreatic cancer. Methods Trials were located through electronic searches of the PubMed database, Embase database, CBM database and CNKI database from the date of establishment of the database to May 31 st, 2012. All the data were searched by two statisticians. Meta-DiSc 1.4 software and Statal2.0 software were applied for statistical analysis. Results A total of ten trials were selected, 690 patients were involved. The heterogeneity was found in these trials except for threshold. A meta-analysis of PCR-RFLP subgroup was performed. In PCR- RFLP group, sensitivity and specificity were 0.714 (95% CI: 0.649-0.773) and 0.868 (95% CI: 0.781-0.930) respectively, area under curve (AUC) of summary receiver operator characteristic curve (SROC) was 0.8544 and Q* index was 0.7853. Conclusion The current evidence shows that the diagnostic value of detection of K-ras gene mutation at codon 12 in pancreatic cancer has moderate effect. It is still inadequate as a separate early diagnostic marker; however, it can be used as an important complement to pancreatic cancer early diagnosis test and an alternative screening method of early diagnosis of pancreatic cancer.
