摘要
目的评价白光联合荧光支气管镜在肺癌早期诊断中的价值。方法将支气管镜检查的患者分普通白光电子支气管镜组(白光组)和荧光电子支气管镜组(联合组),采用刷检和/或钳夹方法取得标本组织送检,对比两组对肺癌组织检查结果的差异。结果入选患者87例,白光组和联合组分别45例、42例,最终两组分别确诊肺癌23例和26例。其中白光组与联合组的一次性检出率分别为73.9%、88.5%,两者有统计学差异(P<0.05)。镜下判断恶性的准确性,白光组的敏感率、特异率、阳性预告值和阴性预告值分别82.1%、71.4%、85.1%和66.7%,而联合组分别是92.5%、46.7%、75.8%和77.8%。结论采用白光联合荧光检查的荧光支气管镜,对肺癌病灶的检出具有高敏感性、低特异性的特点,对气道粘膜癌变的早期检出具有一定的优势。
Objective To evaluate the role of white light combining fluorescence bronchoseopy in early diagnosis of lung cancer. Methods Patients who were appointed bronehoscopy were randomly divided into the white electronic bronehoseopy group (white group) and fluorescent electronic hronchoseopy group (combined group). In order to contrast the two groups showing positive results on lung cancer histology differences, the combined group eheekouted the suspicious lung lesions through taking the white switch fluorescent se- quential inspection methods , and brush biopsy and / or clamp. Results Eighty-seven patients were enrolled and Forty-nine biopsies were found to have lung cancers or moderate to severe dysplasia. The control group made 23 lung cancers, and the ~once detection rate was 73.9% (17) ,while the white light combining fluorescence bronchoscopy group made 26 lung cancers , and the once detection rate was 88.5% (23)(P〈0. 05). In control group,the diagnostic sensitivity was 82. 1%, specificity was 71.4%, positive predictive value was 85.1%, and negative predictive value was 66.7%. Compared with the control group, sensitivity of fluorescence bronchoseopy increased by 10.4% , specificity decreased by 24. 7%, positive predictive value decreased by 9.3% , and negative predictive value increased by 11. 1%. Conclusions Fluorescence bronchoscopy with white combining fluorescent inspection has the advantages of high sensitivity, low specificity, which has certain advantages in the early detection of airway mucosal cancer.
