摘要
目的:提高社区卫生服务中心的医疗质量,减少纠纷和事故的发生。方法对我区五年来发生的17起医疗事故和经浦东新区医患纠纷人民调解委员会调解的99例纠纷进行了统计,分析了社区卫生服务中心医疗纠纷和事故的特点和医疗过失环节。结果:116次纠纷共涉及各类学科116科次,其中学科分布为:内科35次;外科27次;妇产科19次;口腔科16次;中医科12次;预防接种不良反应5次;儿保2次。116次纠纷共涉及过失环节151次,其中前五位过失环节为:漏诊、误诊;告知不全;处置不当;观察不仔细;用药不当。结论:提高医务人员的医疗技术水平、配备必要的急救设备提高急救技能、加强医护人员的责任心、严格执行定向转诊制度、充分履行告知义务、根据实际情况开展适宜技术、规范病历书写,是提高社区卫生服务中心医疗质量安全,防范纠纷和事故的关键。
Objective To improve the medical quality of community health service center and decrease the frequency of medi- cal disputes and accidents. Methods To calculate 17 cases of medical accidents that happened in Pudong new area and 99 cases of doctor- patient disputes that have been mediated by the peopleb conciliation committee among the five years , and to analyse characteristics of medical disputes in the community health service center and and the links of medical accidents. Results 116 times disputes involved all kinds of subjects. The subject distribution was : 35 times of internal medicine,27 times of surgical de- partment, 19 times of gynaecology and obstetrics department; 16 times of stomatology department, 12 times of traditional Chinese medicine department,5 times of vaccination adverse reaction,2 times of Childrenb department. 116 times disputes involved 151 times of negligence links. In which, the top five links were: missed diagnosis and misdiagnosis, incomplete - informed, mis- management, careless -observation and inappropriate-drug-use. Conclusions The key points of improving the medical quality and safety of community health service center and preventing disputes and accidents were: improving medical technology level of staffs, equipping the necessary emergency device, improving the first aid skills, strengthening the sense of responsibility of staffs, strict implementation of directional referral system, fully implemented the inform obligation, carrying out appropriate technology according to the actual situation, standard medical history writing.
