影像学联合叶酸和维生素B_(12)在认知功能障碍诊断中的意义 被引量:1

Significance of imaging combined with folic acid and vitamin B_(12) in diagnosis of cognitive dysfunction

摘要 目的探讨影像学检查联合叶酸和维生素B12在检测认知功能障碍诊断中的意义。方法选取认知功能障碍的患者98例,其中阿尔茨海默病(AD)46例,轻度认知功能损害(MCI)52例。选取同期的健康体检者30例为对照组。分别检查各组血清中的叶酸及维生素B12水平,超声检测颈动脉内膜-中膜厚度(IMT)值,并根据MRI检查对脑白质进行评分。结果 AD组和MIC组的叶酸和维生素B12水平较对照组明显降低(P均<0.01),且AD组的降低水平明显高于MCI组(P<0.05或0.01)。AD组和MIC组的颈动脉IMT值和脑白质疏松积分较对照组明显增高(P均<0.01),且AD组的升高水平明显高于MCI组(P<0.01)。结论叶酸、维生素B12水平降低可能与MCI和AD的发生发展有关,联合检测叶酸、维生素B12、颈动脉IMT值和脑白质疏松积分有助于认知功能障碍的诊断。 Objective It is to approach the significance of imaging examination combined with folic acid and vitamin B12 in diagnosis of cognitive dysfunction.Methods 98 patients with cognitive dysfunction,in accordance with their respective diseases,were divided into Alzheimer's disease(AD) group(46 cases) and mild cognitive impairment(MCI) group(52 cases).30 healthy individuals were selected at same period as control group.The levels of folic acid and vitamin B12,carotid artery intima-media thickness(IMT) values and white matter score with MRI were detected in each group.Results The levels of folic acid and vitamin B12 in AD group and MIC group were significantly lower than those in control group(all P0.01),and the reduced levels of AD group were significantly higher than MCI group(P0.05 or 0.01).The carotid IMT values and white matter score in AD group and MIC group were significantly higher than those in control group(P0.01),and the elevated levels of AD group were significantly higher than MCI group(P0.01).Conclusion The levels of folic acid and vitamin B12 are related with the pathogenesis of MCI and AD,combination detection of folic acid,vitamin B12,carotid IMT values and white matter score is helpful for diagnosis of cognitive impairment.
