小剂量舒芬太尼在剖宫产术麻醉中预防寒战和牵拉痛的临床观察 被引量:83

摘要 目的观察小剂量舒芬太尼在剖宫产术麻醉中预防寒战和牵拉痛的临床效果。方法选择单纯足月妊娠并接受剖宫产术的产妇40例,均实施腰硬联合阻滞麻醉,并按麻醉用药分为舒芬太尼组和对照组,每组20例。舒芬太尼组产妇采用0.75%布比卡因(1mL)、舒芬太尼(5μg,0.1 mL)、10%葡萄糖(0.5 mL)作为麻醉用药;对照组产妇采用0.75%布比卡因(1mL)、10%葡萄糖(0.5mL)作为麻醉用药。记录两组产妇寒战和牵拉痛的发生情况、麻醉中最大感觉阻滞平面和踝关节运动恢复时间及手术时间;记录术前,注药后1、3、5、10min及术毕时的心率、平均动脉压;新生儿出生后1、5min时的Apgar评分等。结果舒芬太尼组产妇牵拉痛的发生率以及麻醉后寒战的发生率明显低于对照组(P<0.05)。两组产妇麻醉期间平均动脉压、心率、最大感觉阻滞平面、踝关节运动恢复时间、手术时间、新生儿Apgar评分、皮肤瘙痒、恶心、呕吐等比较,差异无统计学意义(P>0.05)。结论小剂量舒芬太尼联合布比卡因的蛛网膜下腔注射可有效预防剖宫产术产妇麻醉后寒战,并减少牵拉痛的发生。
