摘要
目的观察熄风胶囊对具有中央颞区棘波的小儿良性癫痫(BECT)的临床疗效及对24小时动态脑电图的影响。方法 37例BECT患儿随机分为熄风胶囊组19例和卡马西平(CBZ)组18例。熄风胶囊组给予熄风胶囊,每次0.99g,每日3次;CBZ组给予CBZ,每日10～20mg/kg。两组均以6个月为1个观察周期。观察两组患儿临床疗效,并于治疗前后行24小时动态脑电图检查。结果熄风胶囊组临床疗效总有效率为84.21%,CBZ组总有效率为83.33%,两组比较差异无统计学意义(P>0.05)。两组患儿治疗后脑电图痫样放电频度在清醒、慢波睡眠和快波睡眠阶段均较治疗前明显减少(P<0.01)。治疗后两组患儿局灶性放电频数均明显减少、局灶性放电部位均有明显改变,两组间比较差异均无统计学意义(P>0.05)。结论熄风胶囊治疗BECT患儿,其临床疗效和脑电图改善程度与CBZ治疗相当。
Objective To observe the therapeutic effect of Xifeng Capsule （wind-extinguishing capsule） for children of benign epilepsy with centrotemporal spikes （BECTS） and the changes of electroencephalogram （EEG）, determine its effective difference with Carbamazepine （CBZ） and find the better antiepileptic drugs. Methods Thirty-seven cases of BECTS from pediatric outpatients were randomized into the Xifeng Capsule group （19 cases） and CBZ group （18 cases）. The Xifeng Capsule group was given Xifeng Capsule, 0.99g once, three times daily. The CBZ group was given 10-20mg/kg of CBZ daily. Both groups were treated for 6 months as one observation course. The therapeutic effect was observed after treatment and EEG was examined before and after treatment. Results The total effective rate of the Xifeng Capsule group and CBZ group was 84.21% and 83.33% respectively, with no significant difference between groups （P〉0.05）. After treatment, the epileptiform discharge frequencies of EEG in awake, slow-wave sleep and fast-wave sleep stage were significantly reduced in both groups （P〈 0.01）. The focal discharge frequency in the Xifeng Capsule group was less than that in the CBZ group, but with no significant difference between groups （P〉0.05）. There were 15 cases （78.94%） in the Xifeng Capsule group and 11 cases （61.1%） in the CBZ group that did not occur focal discharge or declined in the frequency, but with no significant difference between groups （P〉0.05）. There were 14 cases （73.69%） in the Xifeng Capsule group and 11 cases （61.1%） in the CBZ group those focal discharge position changed, but with no significant difference between groups （P〉0.05）. Conclusion Xifeng Capsule is as effective as CBZ for BECTS in controlling clinical seizures and improving EEG.
