摘要
【摘要】目的探讨一次手术治疗尿道下裂合并鞘膜积液，同时降低尿道瘘等并发症发生的改良术式。方法本研究于2000—2011年，对尿道下裂合并单侧鞘膜积液的患者，应用口腔黏膜游离移植再造缺损尿道，将再造尿道近端与原尿道开口吻合，采用带蒂睾丸鞘膜瓣覆盖新建尿道治疗尿道下裂，同时应用鞘膜翻转治疗睾丸鞘膜积液。结果本组共10例患者，均一次手术治疗成功，术后未见尿道瘘、尿道狭窄等严重并发症；术后阴茎、阴囊形态恢复良好。结论本术式可同时治疗尿道下裂及鞘膜积液，并明显降低术后尿道瘘、尿道狭窄等并发症的发生；术后阴茎、阴囊外形良好，有推广应用价值。
Abstract： Objective To explore a modified operation method on the repair of hypospadias combined with hydrocele of tuniea vaginalis, which can reduce the incidence of postoperative complication, such as ure- thral fistula. Methods Totally 10 patients presented to our department with hypospadias combined with unilat- eral hydrocele of tunica vaginalis from 2000 to 2011. All patients were performed the operation by using bueeal mueosa to reconstruct the defective urethra, and then anastomosing the urethra. After hydrocele aspiration, pedicle tunica vaginalis flap was used to cover the reconstructed urethra and sheath turnover operation was per- formed on the rest tunica vaginalis. Results Primary healing was achieved in all 10 patients, who were satis- fied with the esthetic and functional results of the penis and scrotum, without serious complications such as ure- thral fistula and urethrostenosis. Conclusion The surgical procedure can repair two diseases in one operation and achieve satisfactory result of esthetic appearance and reduce the incidence of postoperative complication ac- ceotable complication obviously. It deserves an extensive promotion.
