摘要
目的通过观察盐酸氨基葡萄糖、依托芬那酯凝胶治疗颞下颌关节紊乱病(TMD)的临床疗效,探寻治疗老年TMD的有效治疗方法。方法对2008年7月-2011年12月收治的35例老年TMD患者,予口服盐酸氨基葡萄糖胶囊750 mg,2次/d,依托芬那酯凝胶5 cm局部外涂,3～4次/d,治疗时间为6周,并对治疗前、后颞下颌关节疼痛程度和活动度进行临床疗效评价。结果经治疗,35例患者颞下颌关节疼痛缓解,关节活动度亦明显改善;依据疗效评定标准,治愈10例,显效16例,好转7例,无效2例,总有效率达94.3%;治疗过程中,4例出现轻度胃胀、恶心等不适,2例出现局部皮肤轻度瘙痒现象,无1例中途退出或失访。结论盐酸氨基葡萄糖联合依托芬那酯凝胶治疗老年TMD,能缓解关节症状,改善关节活动度,副作用少,值得临床推广运用。
Objective To explore an effective treatment method for elderly temporomandibular disorder（TMD） by observing the clinical therapeutic effect of glucosamine hydrochloride combined with etofenamate gel on aged patients with TMD.Methods From July 2008 to December 2011,35 aged patients with TMD were treated with oral glucosamine hydrochloride capsule at 750 mg for two times every day and local application of etofenamate gel at 5 cm for 3 to 4 times per day.The progress was 6 weeks.Pain degree and activity of temporomandibular joint（TMJ） before and after treatment were observed,and the clinical therapeutic effect was evaluated.Results After the therapy,pain in all 35 patients were alleviated,and activity of TMJ were also significantly improved.Based on the assessment standards,10 patients were cured,16 got significant effects,7 got effects,and only 2 had no effect.The total effective rate was 94.3%.There were only 4 patients with mild gastrectasia,nausea,and some other symptoms,and another two patients had mild local skin itching during the treatment.No one case was dropped out or lost.Conclusions Treatment for aged patients with TMD by glucosamine hydrochloride combined with etofenamate gel can alleviate the symptom of TMD,and improve the activity of TMJ with few side effects.This method is worthy of being popularized in clinical practice.
