摘要
目的探讨MSCT对胃间质瘤的诊断价值,提高该病的CT诊断准确率。方法回顾性分析经手术及病理证实的28例胃间质瘤的CT及临床资料,并相互进行对比。结果 28例胃间质瘤中,良性或低度恶性18例,恶性10例。主要CT表现为软组织肿块,向胃腔内、腔外或腔内外突出,良性或低度恶性者直径多<5 cm,恶性者直径常>5 cm。平扫肿块密度均匀或不均,增强后良性或低度恶性者多呈轻中度均匀强化,不均匀强化者内见斑点状低密度灶;恶性者实性部分多明显强化,中心见大片低密度坏死。多平面重组可清楚显示肿块与胃壁及周围结构的关系。CT仿真内窥镜能立体地显示瘤体形态、大小及表面情况。免疫组化示26例CD117阳性,21例CD34阳性。结论胃间质瘤的CT表现具有一定特点,MSCT及其后处理技术有助于其定位、定性诊断,可为临床制定合理的治疗计划提供重要的参考信息。
Objective To investigate the value of CT in the diagnosis of gastrointestinal stromal tumors( GIST), and improve its diagnostic accuracy. Methods Clinic data and CT findings of 28 cases with gastric stromal tumors confirmed by operations and pathology were analyzed retrospectively, and compared with each other. Results In the 28 cases, there were 18 benign or low-grade malignant tumors and 10 malignant tumors. The main CT findings were the intra-luminal, extra-luminal or both intra-and extra-luminal soft tissue mass. The sizes of benign or low-grade malignant tumors were usually below 5 cm, and malignant tumors were over 5 em. On the plain CT scan, the mass showed homogeneous density or heterogeneous density. On the enhanced CT scan, the benign or low-grade malignant tumors displayed light to moderate homogeneous enhancement, or punctuate necrosis in the het- erogeneous enhancement. The malignant tumors displayed moderately to obvious enhancement with large necrosis area. Muhi-pla- nar reconstruction could show the relation of mass, gastric wall and surrounding structures clearly. CT virtual endoscopy could ster- eoscopic show the mass' morphology, size and surface. The immunohistochemieal results showed positive for 26 cases of CD117 and 21 cases of CD34. Conclusion The CT features of gastrointestinal stromal tumors are characteristic. MSCT and post-process- ing techniques are more helpful to determine the location and nature of tumors, and can provide important information for drawing up the rational treatment.
