摘要
目的比较内窥镜器械鞘改良腹腔镜与开腹结肠癌根治术的治疗效果。方法入选2009年1月—2012年1月就诊的结肠癌患者278例,随机分为腹腔镜组(136例)和开腹手术组(142例)。观察2组手术时间、术中出血量、术后住院时间、术后排气时间、住院期间并发症发生情况。对所有患者进行随访观察,比较2组患者的累计生存率。结果腹腔镜组手术时间较长(P<0.05),术中出血较少(P<0.05)。腹腔镜组术后排气时间、下床活动时间、进食时间以及住院时间均较开腹手术组明显缩短(P<0.05)。2组住院期间发生切口感染、肠瘘、粘连以及肺部感染等并发症差异无统计学意义(P>0.05)。2组累计生存率差异无统计学意义(P>0.05)。结论内窥镜器械鞘改良腹腔镜下结肠根治术具有创伤小、出血少、恢复快等优点,长期生存率与开腹手术相似。
Objective To compare the efficacy of advanced laparoscopy versus open surgery on patients with colon cancer. Methods All consecutive patients with colon cancer who were admitted from January 2009 to January 2012 were enrolled and divided into laparoscopic surgery group （n = 136） and open surgery group （n = 142 ）. The clinical data were collected and analyzed, including the operative time, blood loss, exhaust time, complications, and duration of hospital stay. All patients were followed up and cumulative survival rates of the two groups were compared. Results The baseline characteristics between the two groups were similar. The operative time in laparoscopic surgery group was longer than that in open surgery group （ P 〈 0.05 ）. The blood loss was less in laparoscopic surgery group （ P 〈 0.05 ）. There were significant improvements in exhaust time, incidences of operative complications, durations of hospital stay in laparoscopic surgery group （ P 〈 0.05 ）. After long - term follow - up, there was no significant difference in cumulative survival rate between the two groups （ P 〉 0.05 ）. Conclusion laparoscopic colectomy is safe and effective for the treatment of colon cancer.
