摘要
目的评价实时超声弹性成像(RTE)五分法与应变率比值法在肝肿瘤定性诊断中的价值。方法对57个患者共64个肝肿瘤进行常规超声、RTE五分法评分,并计算应变率比值,与临床诊断进行对照。结果常规超声诊断肝内肿瘤良恶性的敏感性、特异性、准确性分别为76.6%、70.6%和75.0%;RTE五分法的敏感性、特异性、准确性分别为87.2%、76.5%及84.4%;RTE应变率比值的敏感性、特异性、准确性分别为83.0%、76.5%及84.4%;常规超声、RTE五分法及RTE应变率比值法的曲线下面积分别为0.736、0.819和0.827,常规超声与RTE五分法及应变率比值法曲线下面积Z值分别为0.831、0.936,差异无统计学意义;RTE五分法与应变率比值法曲线下面积Z值为0.087,差异亦无统计学意义。结论 RTE五分法和应变率比值法在诊断肝脏恶性肿块的效能上与常规超声相仿,且二者在诊断效能上亦无明显差异。
Objective To evaluate the value of five grades rating scale and strain ratio of real-time elastography in the diagnosis of hepatic lesions. Methods A total of 57 patients with 64 hepatic lesions underwent conventional and real-time elastography ultrasonography. The elastography images were classified by five grades rating and the strain ratio were calculated. The results were compared to those of clinical diagnosis. Results The sensitivity, specificity and accuracy of conventional ultrasonography for the diagnosis of malignant hepatic lesion were 76.6%, 70.6% and 75.0%. For the five grads scale and strain ratio of real-time elastography, they were 87.2%, 76.5%, 84.4% and 83.0%, 76.5%, 84.4%, respectively. The area under curve of the three mehthods were 0.736, 0.819 and 0.827. No statistical difference was found between five grades rating and strain ratio in the diagnosis of hepatic lesion. Conclusion Real-time elastography can improve the sensitivity, specificity and accuracy in the diagnosis of hepatic lesion. There is no statistical difference between five grades rating and strain ratio of real-time elastography in the diagnosis of hepatic lesion.
