摘要
目的探讨并比较TIC(时间-信号强度曲线)、DWI与MRS对乳腺癌的诊断价值。方法搜集乳腺癌患者31例,全部行动态增强扫描及DWI(b=1000s/mm2)和1 H-MRS扫描,31例均获病理证实,其中导管内癌25例、乳头状癌4例、粘液腺癌2例,统计比较动态增强曲线、DWI及MRS诊断乳腺癌的敏感度、特异度及诊断符合率。结果动态增强曲线、DWI及MRS诊断乳腺癌的敏感度分别为96.6%、91.6%、85.7%,特异度分别为50.0%、57.1%、60%,诊断符合率分别为90.32%、70.96%、58.6%。结论 TIC对诊断乳腺癌的敏感度和符合率比DWI、MRS更高。因此TIC对诊断乳腺癌最具价值。
Objective To assess and compare the diagnostic value of signal intensity time course, DWI and MRS in the diagnosis of breast cancer. Methods Thirty-one patients with breast cancer were recruited. Thirty-one patients underwent dynamic contrast enhancement MRI, DWI(b= 1 000s/ram2) and 1H-MRS scan. The diagnosis of the 31 patients were confirmed by pathology. They were divided into 3 groups: 25 Intraductal carcinoma, 4 papillary carcinoma, 2 mucus ade- nocareinoma. Using integral method and statistics, the sensitivity, specificity and accuracy of Signal intensity time course, DWI and MRS in the diagnosis of breast cancer were compared. Results The sensitivity of signal intensity time course, DWI and MRS for diagnosis of breast cancer was 96.6%, 91.6%, 85.7% respectively, specificity was 50.0%, 57.1%, 60% respectively, and accuracy was 90.32%, 70.96%, 58.6% respectively. Conclusion The sensitivity and accuracy of signal intensity time course was higher in the diagnosis of breast cancer than DWI and MRS. Therefore, the signal intensi- ty time course was most meaningful for the diagnosis of breast cancer.
