摘要
目的探讨血肿碎吸穿刺术治疗老年慢性硬膜下血肿的疗效及其补救措施.方法收集2008年3月至2012年10月收治于昆明医科大学微创神经外科的370例老年慢性硬膜下血肿患者,采用YL-1型血肿碎吸穿刺针在局麻下穿刺硬膜下血肿,继给予闭式引流.结果 370例患者中313例(84.59%),95%CI:80.91%～88.27%,初次治疗达临床治愈;34例,(9.19%,95%CI:6.25%～12.13%,多次碎吸后获临床治愈).因血肿多囊性而给予多次碎吸手术达临床治愈;总有效率达93.78%,95%CI:91.32%～96.24%;对于23例经碎吸治疗无效者行神经内镜单孔直视下血肿清除术,治疗后全部患者同样达临床治愈.结论血肿碎吸穿刺术治疗老年慢性硬膜下血肿疗效满意.
Objective To investigate the treatment efficacy of aspiration punction and remedies for the elderly chronic subdural hematoma patients.Method The data of 370 elderly patients with chronic subdural hematoma patients were collected from March 2008 to October 2012,using YL-1 type of hematoma aspiration puncture needle to puncture subdural hematoma under local anesthesia,then closed drainage were given.Results Of the 370 patients,313 cases（84.59%）with the initial treatment gained clinical cure;34 cases（9.19%）with polycystic hematoma were given multiple aspiration surgery gained clinical cure;total effective rate was 93.78%;To 23 ineffective cases with aspiration therapy,utilizing neuroendoscopy hematoma evacuation under direct vision,acquired the same clinical cure.Conclusion Hematoma puncture aspiration is satisfactory in the treatment of elderly patients with chronic subdural hematoma.
