摘要
目的:分析产科感染的临床特点、危险因素,总结减少感染的预防措施和护理方法。方法:收集2008年6月-2011年3月在浙江省杭州市第一人民医院分娩的4 629例产妇资料,分析产科医院感染发生的部位及相关因素,针对感染发生的部位及相关因素采取相应的护理措施。结果:4 629例产妇中发生感染101例,感染发生率为2.18%,居感染发生率首位的感染部位是呼吸道感染,占33.66%,手术切口、泌尿系统、生殖系统、胃肠道感染、宫腔感染分别占24.75%、12.87%、9.90%、7.92%、5.95%,其他感染占4.95%。医院感染危险因素为胎膜早破、合并基础疾病、侵入性操作、产妇年龄较大、瘢痕子宫、住院时间长、留置导尿管及预防性抗菌药物。结论:产科医院感染的影响因素较多,发病率较高,感染部位多,医院应强化产科感染管理,针对易感因素采取相应的措施,以减少产科感染的发生。
Objective To analyze clinical characteristics and risk factors of obstetric infections,to summarize preventive measures and nursing to reduce infections.Method A retrospective analysis was made among 4629 pregnant women who were enrolled in hospital from June 2008 to March 2011,infectious parts and related factor of nosocomial infection in obstetric were analyzed,and appropriate nursing measures against infections and related factors were taken.Results There were 101 cases of nosocomial infections among 4629 patients（2.18%）.The top infection with the highest infection rate was the infection of respiratory infections,33.66%,followed by surgical wound infection（24.75%）,urinary tract infection（12.87%）,reproductive system infection（9.90%）,gastrointestinal infection（7.92%）,intrauterine infection（5.95%） and other infections（4.95%）.Risk factors of hospital infection were premature rupture of membranes,merged underlying diseases,invasive operation,large cicatricial uterus,maternal age,length of hospital stay,application of indwelling urethral catheter and preventive antibiotics.Conclusions There are many risk factors for nosocomial infections among obstetric patients,the hospital should strengthen the management of obstetric infections,and take appropriate measures in order to reduce the occurrence of obstetric infections.
